Orks have a special rule which allows them to always hit on the unmodified roll of a 6 when shooting. For everyone else, the attack(s) would miss no matter the roll.

Correct. There are rules that say a 1 always fails for Saves and Hit Rolls. There is no opposite rule stating that 6s always hit-at least, not in main 40k.

In Cities of Death, I believe, that rule is added. Same with Kill Team.

There is no cap so if you stack enough modifiers then it is possible to make a unit impossible to hit in some circumstances.

Correct, always hitting on 6 is a rule in Cities of Death, which is a set of optional extra rules for 40k designed for urban combat. It also has some expanded terrain rules.

As the others have said, barring another rule saying otherwise (Cities of Death, Dakka! Dakka! Dakka!, etc.) there is no automatic pass to hit on a 6 like there is an automatic fail on a 1.
