I find the Kill Team Rules additions scummy. Sure it is fine as extras but to consider for competitive play: no thank you. Do one or the other: competitive play without bloat or narrative play with spam.

Thank you for sharing that. Now do you want to explain which rules additions you're talking about exactly?

Most people I've spoken to and seen online just use Core and Elites (and Annual 2019 for points adjustments, but most people just use Battlescribe anyway). That's not all that bad, surely? I ended up buying Characters as well, and love the idea of using it for some custom narratives, but not so keen on it for organised play.

GW themselves have said that Commanders is not for competitive play.
If he's talking about elites then he's dead wrong as it improved balance substantially.
If he's talking about arena then that would be bizarre.
Perhaps he's talking about the team box tactics? Since most were reprinted in elites and all (but not all of the kill zone tactics) were reprinted in the annual the major criticism of those is now removed.

KT could probably do with some units made specifically for the game, especially for Commanders. Some of the Commanders are meant to lead a detachment - even a whole army - yet are given the role of leading a single unit of troops.
For example; An Eldar Farseer traditionally leads a Warhost into battle while a Warlock bodyguard will accompany and lead a unit of guardians or Wraithblade. The Warlock seems about right for Kill Team and a 125 point limit. Tyranids on the other hand have a broodlord who not only guides a swarm but is almost in Carnifex country. A Warrior Prime is still a very powerful HQ but is basically a Warrior that takes point when a larger synapse creature isn't around. The broodlord alone smashes a 125 point limit.

There is definately some room for units that are stronger than leaders but not as strong as commanders that can fit in the core game. Warlocks, lieutenants, etc.

Could be called "Kill Team; lieutenants".

It's not out of character (sorry) in 40k for a captain-level individual to lead a small team on a specific mission, but it works better in my mind for Narrative Play games than for Matched, though. From memory, the Narrative missions in the book all only allow one side to have a character, which can lend to good story opportunities.

5 of the 12 narrative commander missions are commanders on one side only.
You aren't playing only book missions are you? There are so many more missions now... (125 at the moment, of which 25 are narrative or matched commander missions)
Here's a handy guide to all of the missions: https://mega.nz/folder/y7YCTQyY#p3pIGNVx-7lYp6iLnyRtgA
Solitaires are missing, Aradia should never have been a commander, Kroot need a Shaper, etc.
There's a decent amount of material there.

Would love to have a Solitare for Kill Team; bit like the "Ghandi II" of 40K.

Great list of missions...
But it mentions a few that were in the white dwarf but doesn't mention the issue. Any clue where these missions are?
