Some of it is kinda clumpy. Can I just thin it with medium?

Medium doesn't really "thin" the paint, it will still have the same viscosity. What you likely want to do is add water (and perhaps water with a small, small amount of flow improver) since water is the "solvent" (not really, I think it is more of a binding-inhibitor but that is beside the point) in acrylic paints, not medium. Medium is the binder and since your paint is "clumpy" it is presumably already full of binder binding.

Yes.

I add Liquitex Matte Medium to Vallejo paints to get them back to normal. I usually have to stir the contents with a metal sculpting tool to get it to a uniform consistency.

When the paint is really thick, a mix of medium and Tamaya X20A Thinner works wonders.

DON'T use Tamiya X-20. I did & a week later my GW paints were solid.
I advise using distilled water, however I use tap water without issue.
