So my 40k group is finally starting again, so I'm giving the BRB a look through, and there I find in the rare rules the "Fight Again" bit. Does this include Blood for the Blood God on berzerkers/kharn/red butchers, or just stratagems like Fury of Khorne?

If it doesn't, would BftBG be:
(1) Pile In > Attacks > Attacks ("fights twice" > Consolodate
(2) Pile In > Attacks > Consolodate > Pile In > Attacks > Consolodate

Fight again and Fight twice are mostly analogous. The only difference is that if two different abilities both said you may fight twice each Fight Phase, they wouldn't stack, since twice just means, well, twice. Fights AGAIN, meanwhile means an additional time, which doesn't have a numeric cap.

Regardless, in both cases, following (2). Every time you "fight" you go through piling in, making attacks, and consolidating.

Yes, and you can stack the Fight Twice and Fight Again as far as I can tell. For instance, with Khorne Berzerkers you would proceed through the Fight phase as normal, however after you had completed one full sequence with them, BftBG makes them eligible them to Fight a second time, following the normal rules. Please note that they still require a valid target to Fight beyond the first time they do so, as per the normal rules. You do not have to wait for every other unit to have fought once before choosing them to Fight the second time, nor are you forced to Fight a second time with that unit. After you have completed their second turn in Fight phase, you could potentially use the Stratagem Fury of Khorne to make them immediately eligible to Fight again.

Make sure to note that using the Stratagem only makes them eligible to Fight following the normal rules, it does not force you to then Fight with them as it includes the word "can".

Secondary note, even if the unit in question Charged during the Charge phase, the Charging Unit Fight First rules only state that units that did not Charge cannot be chosen to Fight until after all units that did Charge have fought. Though I am not 100% on this, I believe that means that after the first time they fight, that limitation no longer applies and the Fight phase can proceed as normal, even though any buffs from the Charge that last until end of phase or turn still apply in subsequent Fights and they still can only target the unit(s) they Charged.

The way I read it, when a Berzerker (etc.) fights a second time, it is "fighting again" (because it is repeating the "fight" step) and so meets the requirements for the "Fight Again" rule in the Rare Rules section.

Even if this were not the case, BFTBG would still unequivocally be (2). The process of "fighting" covers the whole sequence of Pile In -> Attack -> Consolidate, and they are given permission to do this twice. This means that a Berzerker squad doesn't need to do their second fight immediately after the first (i.e. you can choose Squad 1 to fight, then choose Squad 2 to fight, then choose Squad 1 to fight again, etc.), and your opponent gets the opportunity to interrupt combat in between two fight activations.

Don't forget that a unit can always fight if it made a charge move that turn, even if there are no enemies in Engagement Range. This means that if a unit charges and kills all enemies in Engagement Range with their first fight, they can still be chosen for their second fight and get an extra Pile In and Consolidate move. They wouldn't be able to attack anyone they come into contact with in this way, but it's a good way to "tag" enemy units and try to wrap around them to restrict their fall back move.

It's even more fun when you combine this with the Kill! Maim! Burn! stratagem to get 6" Consolidation moves.
