So I recently made a list and brought it to a GT. I was going 2-1 with completely different lists but then ended up with a huge issue when I went to a GT. I ended up winning 2 games by round 5 and then got paired into the player who placed 3rd due to a bracketing, both my wins were 92, and 99 with losses of 60, 64. At dinner after the GT my friends told me that if I’m winning games and all my losses were within 30 points to focus on what I did wrong and learn from my mistakes. I have a bad habit of drastically changing my lists almost every single tournament.

A good list is only as good as the player controlling it.
List building is an important part of the game ,but so too is actually playing the game and the choices you make. Bad choices can mean even a good list loses; whilst the right choices can mean a "statically weaker" list can still manage a win through superior play and choices.
So yes I would say that you need to balance your list building alongside your gameplay. This might even be just realising that you like/work best playing a certain way and thus building a list that leans on that a bit more.
The rest is reviewing the games and identifying where you make poor choices; what you could improve and what patterns you can see. It might be you're doing similar things each time which set you up poorly for the mid-late game.
Eg I know I tend to be over-cautious and defensive; which can run the risk that I miss objectives further into the game field. This makes them points that have to be fought over or my opponent already has one or two turns holding them before I can at least contest them. So by holding back I'm giving them both a degree of board control and free victory points from objectives.
By moving slower and being defensive and trying to pick perfect fight targets, I'm also yielding part of the control of the game and not getting the most out of my forces.
Realising that fact I'm very slowly (mostlycause games are all spread out over ages) to be more aggressive; to push forward more; to not be as fearful of being attacked when I push forward and such.

Well, the biggest advantage of constantly changing army lists is that people at tournaments knowing you can't predict what you'll bring. Playing constantly the same list makes you more familiar using it, but it also means it's predictable as well. So I don't especially see that as a bad idea in itself. It can be if it doesn't suit you in your gameplay / list mastery, that's all

It depends on what you want. If you want to have fun and enjoy what you're doing, keep it up.
If you want to improve your win rate, play a list for a while or at least don't do any drastic changes between any two games.
Besides tactical acumen and some not too bad dice rolls, you will need to know your list and your opponent lists. To learn your list(s) you need practice. You have to experience what a unit is able to do against what kind of foe and how your units can support each other. Theoretic synergies are nice, but the proof is on the table. So play a list for a few times, before making drastic changes. It should help to increase your win rate. Once you have a understanding what all your units can do, you can go back to making drastic changes between games.

Tailor your list to the missions being played at the tournament. Bigger tournaments will publish the missions. Figure out what suits your play style for the missions. The only thing that can change is terrain.
