I reckon it's not that rare in licensed material - novels in particular have a tendency to make epic stuff seem like an everyday job, simply because just about every book tries to tell another "epic" story, thus making it inevitable that various feats get repeated over time. I suppose the moon blown up by the Sororitas in Hammer & Anvil qualifies as a tombworld as well, anyways.
Did something like that ever happen in GW fluff, though? I'm honestly not so sure now.
Considering the moon in Hammer and Anvil contained a MASSIVE Necron force to the ridiculous scale I'd count it as a tomb world most certainly. Nice crowning moment of Bad Ass for the Sisters and some proper revenge for Sanctuary 101. As to Rule Book/Codex fluff... your guess is as good as mine, but there's lots of things I don't remember from the Codex/ BRB fluff that I just think of as "standard". I suppose it comes down to whether the fluff is canon or not, which GW says it is. So then it's down to whether or not you personally want to accept the fluff. IIRC the "Word of GW" on the matter is that all fluff is canon, not all fluff is true, some of it could be say Imperial Propaganda. Personally the Cain novels, Hammer and Anvil and a couple others are on my "confirmed true" list, but that's just my opinion.

GW never actually touched the term "canon" at all. The closest thing to an official statement was the "is it all true? yes and no, half-truths, blah blah" relativation you were referring to which was printed in both the White Dwarf as well as on Black Library's old FAQ.
A number of individual authors, amongst them ADB, Gav Thorpe, Andy Hoare and Dan Abnett have since made clear that there is no such thing as a canon and it's up to you as the individual gamer to decide what is "real" for your interpretation of the 'verse.
For example, I don't buy into the Cain novels at all - they fundamentally contradict the way I see the Schola Progenium, the Sororitas and the Commissariat working as per the GW fluff. On the other hand, I have adopted some stuff from Hammer & Anvil into my personal vision since it seems to fit in nicely with what I was already familiar with.
But that's why I was asking. From my understanding, most people respect what GW is writing in their books, whereas with novels it very often comes down to individual interpretation and bias. So a destroyed tombworld in a GW book might be "more acceptable to the general public", so to say.

Also, I was told by him that Ghazgkull is the most undefeatable thing in the Galaxy and nothing can defeat; he also told me that Ghazgkull is bigger then a carnifex and can destroy titans and baneblades with one or a few hit and he has single handedly destroyed baneblade/titan legions

When someone says "the life expectancy of a Guardsman is 15 hours", doesn't that probably refer to any Guardsman, anywhere, irrelevant of what regiment is featured in the book?

The "Imperial Guard have a life expectancy of 6 minutes" might be because he's misremembering a peice of Starcraft fluff:

That was a Necron world engine. So literally a Necron equivalent of the death star with void shields that are unmatched by any ship encountered in codex fluff (and it is not hinted at that any BL fluff contradicts this.)

I'm 18 and he's a few years older then me but I usually don't believe anything he says even in non 40k stuff and fluff
On a side note on non 40k stuff, he said that the seven of the old Nine Men of Disney are black and the reason why we think they are white is b/c photos of them are in black/white. That was the thing that made him lose all credibility to me at that point.
