The blood angel codex has some units with as war gear saying "Boltgun or bolt pistol" so thats clear you can only select 1 of the 2.
However with options it says "Replace Boltgun and/or bolt pistol" so here it indicates that it has both.

Should I believe the wargear saying you got 1 of the 2 or the options saying you can have both replaced?

I think it says and/or so you can take the upgrade whether you initially went with bolter or bolt pistol. If it didn't say and/or, they would have to print another entire list with the second weapon and its options to replace. Does that make sense? I could be wrong though...

This. You can switch the ranged weapon no matter which one you took in the beginning.

Sanguinary Guard come with Boltgun and Bolt Pistol as standard. Their option to replace boltgun and/or bolt pistol means you can replace either or both of them with weapons from the list. Twin plasma pistols if you really wanted to.

I thought Sanguinary Guard came with an Angelus Boltgun and Glaive Encarmine.

Perhaps you are thinking of Sanguinary Priests?

I did mean Priests not Guard.

A Captain can do it as well, just for fun, have a captain with two hand flamers join and assault squad with two flamers in a land raider, jum out, flame everything, then assault if anything is left. Another option for the daring is deep striking a captain with dual infernus pistols.
