So, in challenging a unit and the only model that could accept is about 5 ranks back and therefore cannot accept the challenge - is this a refusal? Or does the challenge just simply not go off?

Let's say Urien Rakarth challenges the Nob out of a unit of Boyz. However, the Nob is in the back of the mob and cannot accept. Is he refusing? Or. . .?

As above, nice little technicality. Keeps PK Nobs from getting minced if you pay attention and are keen with movements.

For the first round of combat, after that they are most likely in challenge range.

This. There is a requirement to be Engaged in order to proffer or accept a challenge. If you are not engaged before I10, you cannot accept or offer a challenge

A new favorite tactic of mine if I have a single model engaging an enemy unit (typically happens in the case of a CSM Lord Delivery Vehicle as Chaos characters MUST issue and accept challenges) is to move towards and then charge the end of an enemy squad furthest away from their characters and ICs. Since you are typically striking at a higher Initiative value and he hasn't piled in to engage yet, this allows your beatstick HQ to inflict enough wounds on the squad itself first (without needing to oblige by the challenge rule) such that once the characters get into challenge, you reduce the number of Moral Support re-rolls (which sometimes make all the difference in challenges!) that he can make.

Well technically you can offer a challenge with an engaged character, but if no character in the opposing unit is engaged there would be no one to accept.

This can be risky if your characters kills enough models that you can't pile in - so be careful.

If it's the only model assaulting, you're fine. Actually, it's probably more advantageous in some circumstances. You've piled in and struck at your Initiative value, so if the opponent can't pile in enough units (e.g. only 1), all the better to you. By the time the next phase of combat rolls around, you've piled in another 6" (end of combat from the previous round 3" and then your 3" in the next round at your Initiative step)

I'm not sure to understand the tactic you proposed... You charge out of challenge range and then you get in a challenge? how is it possible? probably I'm missing something.

In later rounds, when you get into challenge range (and have to challenge, as a chaos character), you have killed enough of the enemy non-characters to stop / reduce their "by stander" reroll.
