The smite mode is +6 strength AP 2, one use only. Does this mean when using the smite mode that you only get one attack?

Unless stated otherwise you get to use the same amount of attacks as normally.

no, just that you only get to use smite once per game.

No, it means you can use Smite Mode once only. You then have as many attacks with it as you would expect

Same as a combi-plasma still fires 2 shots at targets within 12"
