There's something I tell people about this game that, ultimately, I think holds true: A model worth 4000 points, and an army worth 4000 points, are not equally powerful. Or, to put it another way, power and point cost do not scale perfectly.
To prove this to a group of friends, I decided to Make a single unit worth 4000 points and let them play against it with an army of the same value. To do this, I took the rules for a Titan, and attached them to an infantry.
My problem is that I'm still short roughly 1500 points of my mark. So, I decided to make up a few of my own rules, but... they're not working. So, I'm looking for suggestions. How would you all go about making a titan 1500 points more expensive, keeping in mind that it's an infantry-sized model.

Err, I'm not really sure what your question is. 4000 points in one model MUST be equal to 4000 points in an army, that's the whole reason for having a point system. If you find that your 4000 point infantry model is always worse than the 4000 point army then that just means that you screwed up the point costs on the single model and really had a 2-3000 point model, not that 4000 points is worse than 4000 points.
As for making one, I have no idea. A 4000 point model is so far outside the normal game that all you can do is try a bunch of ideas and playtest them until you find something that is balanced. Just keep adding D-weapons until you win.

Ultimately, my point has always revolved around one idea:
If a 4k point model was ever really made, then the only way to represent it would be to make something that would, effectively, be unstoppable.
Short of giving him what amounts to advanced void-shields and a re-rollable +2 invulnerable save, though, I can't really think of a good way to represent this. After all, 4k is a Lot of points, and something worth that much should not be easy to kill... to the point that I'm trying to think of a rule that basically makes it so it can't be killed in not only a single attack, but in a single turn. It's... not easy. And writing fluff for it is insane, as I'm trying to stay away from just making it a deity.

Well yeah, there's a reason why even primarchs are only 5-700 points. You haven't discovered some fundamental truth that a single 4000 point model is worse than a 4000 point army, you've just discovered that a 4000 point infantry model in 40k is just plain stupid.

|
Front line gaming used a house ruled imperator titan against a lot of points of enemies. I can't remember the outcome though, or if it seemed balanced..
