This is an article from 2010 about a woman who has no fear response as a result of brain damage. I knew I had to post it here as soon as I read the first few sentences: A 44-year-old woman with a rare form of brain damage can literally feel no fear, according to a case study published yesterday in the journal Current Biology. Referred to as SM, she suffers from a genetic condition called Urbach-Wiethe Disease.
Emphasis added. SM, huh? How perfect is that? So now we know why the SM know no fear -- they all have Urbach-Wiethe Disease. It probably also explains why they live according to a strict monastic lifestyle, given the attendant inappropriate social behaviors.

Batman dealt with it once. It didn't go over well for him having no fear-he almost died multiple times. I realize it's fantasy vs this reality, but I can't see the results being much better. As he put it: "A little fear is a good thing."

Reality is a nice place to visit, but I'd hate to live there.

If she has no fear she cannot know courage.
So long as she is realistic enough not to tempt fate having no fear can be a good thing.
As for the Astartes perhaps surgery simulates the condition, its thr sort of thing the Imperium would do.
