Many, many I'm sure. I think the worst has to be in a 6000 point not-apocalypse game.

SW, UM, BA and GK (All me) against Orks, Tau and Crons. The GK contingent consists mostly of a 10 Paladin 700+ point deathstar, and as we were playing long-ways across the table I figured it would be faster to DS them in...

Not only do they not arrive until T4, but they scatter on 30 orks. I roll mishap, and it comes up 1. 700 points and my last hope down the drain.

I remember my first game against Dark Eldar, I only read their old book, so didn't know any of their new rules. My group house rules Warlord traits to roll a die first, and then pick the result from the 3 charts.
I rolled and picked Night Fighting. Wow, cover saves! this could really help me!

Opponent says NOTHING to me. Starts the game. "Oh, Dark Eldar have Night Vision on everything..."

I was pretty much tables mid turn 3.

I did something similar in my first game against Dark Eldar a couple weeks ago. He rolls the warlord trait to pick whether or not Night Fighting is on, I don't realize everything his whole army has Night Vision (and stupidly, that everything that I brought had Blacksun Filters), and deploy wicked aggressively on Hammer and Anvil deployment. He measures out 42" (36" plus the 6" move so I couldn't see anything) and puts everything out there. I sat around useless on my turn 1 then proceeded to get blasted to after his turn 1 he annihilated the key bits of my army and I spent the rest of the game trying to recover without markerlights or a buff commander to help my BS3 hit anything. Result of inexperience with his army, my army, and just a general over-aggresiveness that I'm trying to get over from transitioning from Tyranids to Tau.

Not understanding the rules for Deepstrike in 5th Ed. As I recall...(this was ~ 6 years ago).

My squad of Terminators deepstriked, scattered and suffered a mishap and were "Misplaced".

My opponent chose to deploy them onto the roof of a tower which we'd agreed was impassable terrain. And because it was impassable terrain, he argued that the unit was destroyed. I later learned that he was cheating, because even when "Misplaced" a unit must be deployed into a valid location following the normal rules for deepstrike. You cannot deliberately deepstrike onto impassable terrain, ergo he should not have placed it there.

It was a small 750 pt game or so, and I was using the Black Reach starter set (Captain, tactical marines and terminators), so those 5 Terminators were half of my army.

In my defence, I was a 17 year old newbie to 40K and he was a 20 something "Veteran".

The worst gaming mistake I made was going to a few tournaments when I am not that type of gamer. It was no fun fo rme, and probably no fun fo rmy opponents.

- Throwing my Salamander Captain with a power sword against an Ork warboss... with a power claw. Keep in mind, this was back in 5th when power weapons completely ignored armor saves and a Power Klaw was not unwieldly but still did attacks at double strength. So my only advantage was being able to ignore a 6+ armor save while my opponent was able to land nearly every blow.

- Forgetting to shoot on my turn. Half my force was destroyed afterwards.

Back in the days of MK1 Warmachine I was playing at Gencon Aus on the finals table vs my best friend. I was the reigning Australian Champ at the time and this game was extremely close. My friend had only a single solo contesting an objective, hidden behind a building I moved up my own solo, to the other side of the building to continue contesting and moved all my other forces to help win another board area. The building was suitably large that even flat out there was no way his solo could get to mine and I was happy to leave that one a tie and win other objectives.

Alas that solo was a Skorne Extoller Soulward. Known in Mk1 for it's ability to shoot through obstructions at things it couldn't see. All I had actually done was move my solo into range to be executed and give up the critical point that cost me the game and a second Jackhammer trophy when time was called.

Warhammer 40K - 1.5 edition (with the vehicle templates from White Dwarf).

Dark Angels, advancing, using a Predator as cover.

First shot of the game targets the Predator, which blows up with a Str 10 pie plate.

The entire army was killed by that one shot.

Deadzone - Plague, gathered behind a building, planning to hit the enemy next turn.... But the enemy lobs two hand grenades over the wall, and kills almost all of the Plague in that one turn. (Totally worth it! That was my girlfriend hucking grenades at me! I was so damned happy - she kicked my arse fair and square!)

Most recent game of 6th (think it was a week before 7th dropped) was a double game, friend's eldar and my guard against my other friend's demon's and yet another friend's DA.

Set up my force, first time I've played the new Guard dex so I'm stoked, I took my wyvern, Pask with two Punishers and a bunch of infantry.

And completely forgot to wrap my tanks with the 40 man platoon I took.

Turn 1, DA bikes fly up, assault Pask and his tank buddies with melta bombs and a melta gun. Punisher explodes, Pask explodes and a glance is put on the last Punisher.

I started laughing when it happened, and my friends thought I'd gone nuts given how I'd just given up my Warlord and half my heavy support.

Worst gaming mistake? I bought the core set and an intercepted and A-Wing for the X-Wing game. Go see the thread on "your fleet" to see how that qualifies.

Reality is a nice place to visit, but I'd hate to live there.

Infinity, recent game.

Using YAMS objectives. I'd completed 3 of 4.

Last turn, Tarik Mansuri waiting eagerly to sprint across the table and make up the 3rd model across the center line. He leaps onto a nearby building. Leaps down onto the adjacent street, hammering an Aragoto with his Spitfire mid-air.

He lands, sprints around the side of a building into cover, safe to claim me my 4th objective.

Until a camo marker start moving in my brothers turn. It rounds the corner, and he reveals a Raiden with Spitfire who hammers Tarik in the back. I fail 2 of the 4 saves, pushing him Unconscious

I really should have used my own camo'd Hawwa to uncover that guy earlier. That onedecision to leave it alone cost me the game. I lost 4-3. That Raiden claimed his 4th objective, to kill my most expensive unit.

Off the top of my head, I think a game I played a while back, my Tau vs. Dark Angels, using the Scouring mission. Opponent was running Terminators and those plasma bikers (no idea what they're called), plus a bunch of small scout squads to fill out his Troops to hold objectives. I get the first turn, and he's hidden all of his scouts behind LOS-blocking walls (and everything else is in Reserves), so I basically spread my forces out and do a big jump move on my Riptide to get into range of the Scouts, figuring that the next turn, I'll start taking them down one unit at a time.

Turns out, not a smart idea on either part. His first turn kicks off, he brings in 20 Terminators, and in my rush to get away from them, he manages to split my forces down the middle. He assaults my Riptide with his Scouts, and for the entire game, I'm stuck in combat, and I'm convinced it's because my dice decided that they weren't giving me anything that game. I get 3 attacks each turn, and most rounds, I either miss with everything, or I'll get one hit and roll a 1 to wound. Meanwhile, bad terrain means I can't maneuver very well, and his Terminators just start wrecking things. I drop in Farsight to take out his unit of Devastators (and try to take out the scouts to free up my Riptide), he brings on the bikes and nearly wipes them out with plasma.

In the end, I finally (barely) managed to get things under control after I lost just about everything, and I figure that I can pull off at least a draw - I've got him down to a single squad of Terminators, and the only objective I hold is a 3 (and his Terminators can't possibly get to anything but another 3 by the end of the game). I comment on that, and he mentions that his bikes are scoring, because they're fast attack. So, of course, he gets to the 4 objective with the bikes and parks on top of it, and I take a big loss because I'm a complete moron. The whole game was one giant on my part.

My friend has 3 of his craft worlds avatars fighting an endless battle in some alternate reality because he keeps getting them get sucked into the monolith portal of another player.

I mean the odds are nuts for that, let alone repeating the mistakes. So thats among the worst mistakes made in 40k gaming I have witnessed over the course of a few months.

I accepted a challenge from the local champion, WHFB my Skaven VS his chaos Dwarfs.
we rolled up that 'divide your side into thirds and roll randomly to see where your units deploy. I end up with most of my Skaven packed into this little corner facing most of his army and with a large rock pillar blocking my advance. His army was a gunline army.

After three turns 8 out of 200+ Skaven finally made it in to combat.
