Does flank give an extra damage die on ranged attacks, such as those from assault? How about impact attacks (like when Steelhead Cav charge into models engaged by halberdiers)?

It's got to be a melee attack, so that's a no on the assault shots. I've still never actually bought/used cav, so I haven't looked into their interactions too much. But I can help with the first bit of your question, and its a 'no'.

I assume this relates to the Steelhead Cavalry?

Flank specifies Melee attacks, so ranged attacks, even if done from melee range using assault, do not benefit.

Impact attacks are Melee attacks, however, so they do benefit from flank.
