I still think my favorite was a GK Lib casting Vortex killing a bunch of stuff. then after getting locked into combat with the enemy chaos space marine warlord for a round, the thing swung back and ate both of them. then disappeared.

I've a friend who plays Orks and is a big fan of the Shokk Attack Gun which is, lets face it, is this thread personified. Will it murder all of my terminators, suck a goodly proportion of his army into the warp or teleport his mech into close combat with my dreadnought?

I used to play a triple Veil Necron list under the 5th codex (Obyron and 2 Harbingers).
The sheer number of 'relocated' mishaps that got rolled... heh.
Actually, in the most memorable case, it was my opponent who self destructed. My Royal Court mishapped, "other player may choose where to deploy the unit."
So the other guy deployed my Royal Court right in front of his Deathstar so he could charge it, knowing that the Court had very little shooting and that it was all about 6"-12" range. He put me 13" away.
Then he charged me the next turn, expecting an easy win, only to get hit by the full anti-deathstar counter-charging monstrocity that was a full Royal Court with Lightning Field, Seismic Crucible, three of those AP1 flamers, five Lords with MSS and Warscythes, Zahndrek and, of course, as a result - Obyron.
The Cruciple meant that he only got five of his deathstar into combat because the others couldn't reach. Let me pick exactly who got Mindshackled. It was a slaughter.

I've stopped using my inquisitor's psychic henchman for anything but a battery. I love my model for him and labored to paint him well. I rewarded with a guy whose manages to perils every time and usually takes a chunk of the squad with him. So much Heresy.

I have one Guardsman in my Imperial Guard army that I refer to as "Private Butterfingers". He is a Veteran armed with a demolition charge. Every time I use him, without fail, he succeeds in dropping his demo charge at his own feet and blowing up himself and his squad, rather than the enemy.

Space wolf opponent used Jaws of the wolf world(whatever its called) shot the beam line across them, which actually landed on most of the big nids,(I was running around a hill so they were lined up as strait as could be) they ALL failed their initiative test, and died. Lost 1400 points instantly.
7. playing as tyranids with a unit of 30 termagaunts, my opponent used the slaanesh ability to move a unit with both of his Daemon princes(5th edition), he managed to move them into dangerous terrain, which I rolled miserably to save them, and lost 23 of the 30 Termagaunts.
For 4, why didn't you Hit and Run? Sammael is Initiative 6, no? :/

There was one time a couple weeks ago, I was in a 6,000 point apoc game, I had a massive death star with 2 overlords 4 lords a destroyer lord and 10 lychguard. They come in turn two after a brutal alpha strike and immediately scatter in to a stormsword and die, at the end of turn 1 I had lost 3000 points in a 6000 point army.
With my guard, it is just down right ridiculous how many tanks I have lost due to overheating plasma cannons.
One game against dark eldar I had an executioner go to fire, charged up its sponsons both over heat but I make one save so 2 HP left, OK fire the main gun all three shots over heat only make one save and bam, my tank commander warlord is dead turn two with no effort from my enemy.
I also had a 1250pnt game against a riptide wing, at the top of turn three my tank commander was down to one hull point and his buddy had two hull points, both were vanquishers with plasma sponsons. Riptides were eight inches away so I had to move away or be assaulted guaranteed. I go to move my TC through difficult terrain as I had no choice and of course I roll a one, dead TC. So then in a "glorious" last stand his buddy turns to unload in to a riptide with only two wounds left, main gun misses, Las cannon is saved and then both sponsons over heat and that tank gets wrecked as well.
I also once had a veteran guardsmen squad blow up the chimera they were riding in by popping out the top hatch and spiking the demo charge against the chimera's hull, then they got pinned from the explosion, but as least only two of them died.

Should have added I tried to hit and run but he always rolled a 1 or a 2 that game, Opponent was rolling real hot that game. It was Laughable terribad. Looking back now I treated it as both players add their initiative to their roll to see if they won or not(huge error of judgment there lol)

- Fail leadership with an 11. Unit bails.
And they shall no fear - roll off the board - kill point given to opponent.

My renegade blood angels include a chaos lord with jump pack leading a unit of slaanesh raptors with 'apothecary' (6th man with plasma pistol and icon of excess).
They'd always turn up at the end of the game and WITHOUT FAIL the 'apothecary' would kill himself with his plasma pistol. The chances of it happening once are slim, but to every time roll a 1, 1-2, 1-4 is impressive. I just guess this apothecary wasn't happy with his companions choice to turn to the spikey side!
Now I am planning to reinforce the squad to 11, which means he'll be rearmed with a bolt pistol and I'll have another melta as my second 'special weapon guy'. I'll probably downgrade the champion's powersword to a CCW and use the old lord as the new squad champion (making 12 raptors with 2 melta, combi melta/lightning clw and FNP).
It's a fluffy army, but I want to open up more HQ slots and the lack of punctuality has cost him his spot as part of the warband's head

Tyranids v Dark Eldar. I had Hormagaunts tarpitting a Chronos (or Talos, I forget which. The more melee focused one). My Biovores shoot at something semi-nearby, scatter 8" off, directly into combat. 7 Hormagaunts dead, not a scratch on the MC, combat was over that turn. They did keep that thing there for about 3 turns though.

Never happened to me, but I've seen it. In 3rd edition, Ork Lootas chose one of the following units and had their weapon options - SM Tac Squad, Dev Squad, Scout Squad, IG Infantry, Fire Support or Anti-tank. (Alternatively yo could use any army list entry as long as your opponent agreed). Lootas were 5-10 models. If they rolled a 1 To Hit, the shot hits their own unit. So imagine this. A mob of 8 Lootas. 4 of them have Plasma Cannons. The 4 plasma cannons all roll 1's To Hit. Now not only do they get hot, but the remaining 4 Lootas are getting hit with a S7AP2 shot. From their own squad. Goodbye Lootas.

Greebo had spent an irritating two minutes in that box. Technically, a cat locked in a box may be alive or it may be dead. You never know until you look. In fact, the mere act of opening the box will determine the state of the cat, although in this case there were three determinate states the cat could be in: these being Alive, Dead, and Bloody Furious.

I've lost my juggerlord with axe of blind fury to rolling 1 for the daemon weapon on a number of occasions.
Had a particularly funny one recently. My regular opponent wanted to try out his new Shadowsword (? - Baneblade with Str D cannon) so we had a silly battle with super-heavies and I got my Greater Brass Scorpion out.
On his turn 2, the GBS took a hit from the D cannon - rolled a 6 on the damage table, then another 6 for 12 hull points. It then went nuclear and I rolled for the highest possible damage on its 'blowing up' table. So my Daemon Prince, hiding behind it, also took a Str D hit and died. As did the remnants of a squad of Havocs hiding in nearby cover.
Tabled on turn 2. Half my army never even arrived on the table. Laughed my ass off - I was tired and was looking for a quick game, but maybe not quite as brutally quick as that!

We were playing a Cities of Death game with killpoints. He was playing chaos. He tried to summon 8 squads of bloodletters over the course of the game. All 8 scattered and exploded from Deep strike mishaps. Our team won only because he tried summoning.

My girlfriend had some bad luck Deep Striking back when she played Chaos.
She brought the 3 Helbrute formation, and liked to placed them right next to enemy units, of course always scattering onto them and mis happing.
Then one game she had a Sorcerer in Terminator armor with a 5 man squad of Nurgle Termies come in from Deep strike, scatter, mishap, rolled a 1.
then, I was playing a game against a new guy at our store who was running Blood Angels. He had a Terminator Librarian, cast 2 powers, Perils on both, and gave me First Blood and Slay the Warlord.
As far as stuff happening to myself, I don't use psykers, and and I don't play risky with Deep Strike, so I can't really kill myself that easily.

My best self destruct was when I shot a Vindicator at a squad of Terminators encroaching on its postition, merely 6" away I figured, "Shooting fish in a barrel" until I scattered 10" Back and nailed my own Vindicator.... That Pilot was clearly drunk.

Back in 6th edition, I had just recently bought a keeper of secrets and my wife painted it up for me, so I was really eager to play with it. I played a 2000 pt game vs space wolves, turn 1 my opponent pods in wolf guard terminators and tries to shoot down my bloodthirster with plasma, who jinked all the wounds away. Perfect, I thought, seeing his terminators well in range of my keeper and my bloodthirster for a first turn charge.
I charge both monsters into the wolf guard. Hammer of wrath, nothing. Keeper of secrets whiffs hard, causing one wound which gets invulnerable saved. Bloodthirster swings... misses everything. I take 2 wounds from thunder hammers.
End of combat, 2 instability tests to make... bloodthirster first, rolls 12... dead. Keeper of secrets tests... rolls 12. Dead. I could hardly believe it. I lost 600+ points to the warp in my own combat phase, in a combat that should have been an overwhelming victory for me.
Those wolf guard did more damage that game than I've ever seen, just by causing me to nuke myself. I lost that game quite handedly, and probably should have just conceded.

One of my friends charged one of my IG sections with his Vanguard Veterans, all tooled out and kitted up. His sergeant (kitted out with all kinds of stuff including a double handed relic weapon) challenged mine to a duel.
Picture this, a great golden titan of armoured perfection wielding an ancient blade forged to cut through all in his path comes swooping in on golden wings. In a voice that makes the foe weak at their knees he issues forth his challenge and swoops down to destroy the mere mortal whom dares defy him....
Only to have the humble infantryman shout "feth that" and blow his head off before he can land a blow.
Yes, my unequipped IG sergeant killed a fully kitted out Vanguard sergeant in a 1v1 challenge.

1. Not so much a self destruct, but just pure dumb luck. My trukk with ram decides to tank shock a riptide. He decides to death or glory, uses his tl fusion thingy and gets an immobilized result on my trukk. The room is building with anxiety as I roll my ramshackle....it's a 6. What happened next could only be described as an ork waaaghing his head off while trying to tbag a advanced tau walker with his trukk.
2.while I have sag storries, my most memorable was the telly port blast on my mega armor big mek. After insta deathing a riptide with it. I decidedicated then to turn the very same weapon onto the buffmander, just to drop that blast on my bigmek warlord, a squad of 3 meganobz, and a painboy. Nothing survived. The real tragedy was I forgot I had an ammo runt for reroll on the blast
