There are so many things in Warhammer 40k so f#$^ed up, it would give H.R. Ginger nightmares. But the thing I think stands out the most is the Daemoncubla, the concept of them alone is sick (women are used a birthing wombs to give birth to Chaos Space marines, and that's the nice version) there is a artists interpretation of it but be warn if you look it up, it is the most morbid and f#$ked up art I have ever seen for 40,000. So how about you, what do you think is the most morbid thing in Warhammer 40k?

Aside from what you mentioned, the whole Genestealer cult thing. I'm pretty certain that it means they were just having... breeding... things happening down there, yeah.
It doesn't ever go into much detail about what happens when different generations crossbreed or genetic relatives crossbreed, but I'm pretty sure that's how you get the abominations.
That's kind of 'The Hills Have Eyes' and it creeps me out.
EDIT: In our 'Black Crusade' campaign, we lost a 'cult' network on one world and we found out they were taken over by a Genestealer cult. We're all of different legions/warbands, but we zealously exterminated every individual we found in their underhive area, and ended up realizing how far the cult stretched. When Chaos Marines are saying, "How can we get the Imperium to Exterminatus this planet?", you're pretty twisted.

This is up there. If I remember right the CSM also came out with no skin so they were harvesting skin from people to sew on them once they came out.

In fairness, I think the haemonculus stuff is probably the ideas that Barker left on the editing floor. I love that man's twisted imagination.
I'd probably say the sacrifice of 1000 people a day to maintain a corpse. Not much that is sicker than that.
As I always say in these types of threads. It the body horror ones that come up. The Daemonculbula has already been mentioned but we also have what Huron Blackheart did to a former Sorcerer of his, Sartak.-

Sartak was once a former Librarian of the Astral Claws but later became a Chaos Sorcerer of the infamous Red Corsairs. Sartak felt that he had followed the easy path for far too many years. Sartak remembered those dark days on Badab, when Huron had poisoned the Astral Claws against the Emperor. Sartak, loyal to his Chapter Master, as a Space Marine should be, followed him into heresy. Along with 200 other battle-brother he escaped his destruction when he fled Badab Primaris at the end of the Badab War.
But the years of reaving had taken their toll on the once idealistic warrior. Like a sleeping man jarred to consciousness, Sartak had opened his eyes to the depravity and corruption of the man once known as the Tyrant of Badab. With this shocking awakening Sartak had realised that there was only one way to make good his betrayal of the Emperor.He had been found unconscious on the bridge of a White Scars’ cruiser following a Red Corsair raid by the White Scars battle-brother, Arghun. Sartak claimed that he was allegedly through with being a blood-handed reaver and had betrayed his oath of fealty to the Tyrant. If was allowed to infiltrate and find his friend Lothar on Blackheart’s flagship, he would be able to tell the White Scars where the next attack would fall. Brother Arghun and Sartak make their way into the Maelstrom and are eventually captured by the Red Corsairs and brought before their Chaos Lord.
The Tyrant queries Arghun for the reasons he has betrayed his oaths of fealty to the Emperor. Unconvinced of the supposes traitor’s intentions, he orders Sartak to slay the White Scar in order to prove his loyalty. Reluctantly, Sartak does as he is told. After the execution, he seeks out his contact aboard Blackheart’s ship – his friend Lothar. Unfortunately, he finds Lothar in a deadly gladiatorial contest against the reigning Khornate Champion, Crassus. Hopelessly outmatch, Lothar is summarily beaten. As he lay dying, Lothar informs his friend of the Red Corsairs’ next target – the planet Razzia. Retreating to his quarters, Sartak reaches out to the aether of the Warp and finds the mind of a White Scars Storm Seer waiting for him. Sartak then informs him of the Tyrant’s next target.
After sending his astrotelepathic message, Sartak is confronted by Garlon Souleater, the Tyrant’s chief psyker, as well as Blackheart himself. They inform Sartak that they were aware of his betrayal and that they had used him to misdirect the White Scars for their next intended target which was the Imperial world of Santiago, not Razzia. Facing his doom, Sartak attempts to attack Blackheart but loses both his hands to the deadly Tyrant's Claw for his trouble. Garlon then psychically attacks Sartak and knocks him unconscious.
When Sartak finally reawakens he finds himself confined in some kind of container and in utter darkness. His limbs are restrained and tubes have been shoved down his throat. He tried to cast his mind into the Warp, but found that his powers had been suppressed. Sartak comes to the bleak realisation that he might be sealed into some sort of escape pod and jettisoned into space. But much to his horror, he comes to the sickening realisation that a worse fate has befallen him. Horrified, he realises that he is sealed within a sarcophagus of a Chaos Dreadnought, never to be installed into the nigh-indestructible adamantium shell of a Dreadnought body. Kept barely alive, he is doomed to slowly rot for all eternity. With this shocking revelation Sartak’s sanity flees his once rational mind in a silent scream.
Personally I find how people are viewed and used some of the most morbid facets of 40k. Dessicated humans hooked up to amniotic pods to power machines as they waste away. Lobotomised servitors. Psykers viewed only as power sources. The body horror is gross, but it occurs so often that it loses its edge and becomes almost normal. The horrors inflicted on people in the name of servitude are much more grounded and disturbing for me because they feel much closer to a possibility.
That being said, the Golden Throne becomes a lot less horrific when you consider that tobacco kills 6 million people each year (according to the CDC). 365,000 annual sacrifices to ensure safe travel and prevent the total collapse of a galaxy-spanning empire seems pretty reasonable in comparison. :p

No dark Eldar is allowed to kill any of them, but all are allowed to torture them.

You are a young male about the age of 12-14 and you just finished slaughtering many of your peers (possibly your friends or even family) and you are taken away by these giants from beyond the stars, explicitly knowing you will never see anything from your old life again.
You are taken into a chamber and one of the giants is ordered to cut you open and install the "divine organs" to turn you into one of them. No anesthesia, no comforting words. And they will do this 19 times over the course of *years*. The surgery itself is horrifying; again, no anesthesia and they're basically going medieval on your body while performing complex surgeries that involve cutting bones, sewing up organs and arteries, reconnecting nerves, and so forth. And some chapters have prayer breaks in between doing things, so badly that some initiates require reconstructive surgery afterwards. The final one of these essentially involved them flaying the top part of your torso (again, fully conscious), peeling back the skin, attaching a film of black, plastic-like substance that connects to your nerves, and then re-attaching your skin over this.
And after all of that? You get to being your career as a scout. The lowest on the totem pole of space marines. You have endured the complete loss of your family and former identity. You've killed a hundred of your peers, most of whom you likely grew up with and played with during that very short childhood. You are then subjected to 19 very traumatic surgeries while enduring very harsh "rehabilitations" in between. All the while you are just a kid on the cusp of puberty. And then you realize this is the bar of entry for Space Marines. Every single space marine that suffered an inglorious death, every one that died to a stray bullet or were just unlucky had to endure this. This is not the act of a vengeful enemy, the experiment of some amoral mad scientist, nor the punishment for some exceptional crime. This is the state-sanctioned procedure for the recruitment of one of their armed forces, one that (while relatively small on paper) numbers in the millions.
It's not helped by the fact that this is the *standard* procedure. Other chapters have even more heinous acts like the pain gauntlet or being locked inside a sarcophagus full of blood for a year in a coma (with the very real possibility of waking up long before a year is up and having to endure it for the remainder with no one able to help you out). And then there are the failures; people, children, who have endured all of this and failed at some point, now destined to, at best, be a chapter serf and constantly reminded of what they could have been, but now have permanently lost. Worse is to be mindscrubbed and turned into one of the many servitors that keep the Chapter Monastery running, or to degenerate into a feral beast, useful only for target practice by your former potential battle brothers.

Mind you, your (well written) account sounds an awful lot like a mish-mash of the very worst each chapter has to offer.
I count at least four very chapter specific parts of the process.

A lot of chapters change some sequences of it, but all of them require the 19 implants (unless they've lost one of the glands) and usually there's no anesthesia. I think Blood Angels do sedate their initiates, but they're the exception rather than the norm. They also always choose a young male between the ages of 12-14, as the organs are basically tweaking with your growth spurt to change you into a marine.
While the "beating up your peers" thing is not something they all do, the alternative is rarely better. Salamanders, if I remember, have to basically re-enact the challenge Vulkan and the Emperor did when they first met (as in, find and kill a huge-ass Salamander) while a lot of others requires reaching the meeting site alive. Sounds simple, but getting there is basically trying to dip your balls into a vat of hungry piranhas and praying for the best. In fact from the ones I've read, killing the other aspirants is actually one of the more tame methods.
Ironically, it's the failures that vary drastically. Ultramarines are the ones who retain their failed as serfs if I remember (because we have to have at least one chapter that isn't comically horrible) while Dark Angels are the ones that mindscrub you into a servitor. Black Templars are the ones that ends up using failed initiates as sparring partners/target practice I think, but I got that out of the comic book.
Most of this I got from the older editions. I'll see if I can cobble together some sources for those that wanna delve deeper.

I actually thought you were referring to the Gates of Morkai challenge where Space Wolves initiates get dumped naked and unconscious in the mountains, a goodly portion of them fail, lose their minds and turn Wulfen then try to kill the other initiates.
Being butt naked target practice is a bit of a stretch but there's definitely some wild sparring going on.
I don't break the rules but I'll bend them as far as they'll go.

For all the air and graces the Necrons have, they gave some pretty messed up stuff in their fluff. "Highlights" include:
- Causing and entire star system to wither and die out to permanent darkness and cold because the Necron race didn't like the inhabitants so they stole their sun.
- Illuminator Szeras being essentially Josef Mengele, doing experiments/vivisection ON STILL LIVING BEINGS. The kicker? He keeps them in suspended animation once finished so he can carry on, with no medical or pain relief. And if they scream too much during procedures, he simple turns off his hearing unit.
- Not all Necrons wanted the bio-transference. So, ghost arks would roam cities, beating up non compliant citizens, and drag their half beaten forms to be 'Necron Warriord'.
- Gauss weaponry essentially causing you to disintegrate from the point of contact. And the Deathmarks rifle is especially a barrel of laughs - doesn't cause initial damage but melts your brain then your body. Nice.
- Mindshackle scarabs causing horrific pain so much do that it may cause you to blow your brains out due to pain delirium. And Bloodshackle scarabs are just as nasty, little homing beacons that attract flayed ones that lock onto your blood, so you can't hide from being chopped up, skinned and your flesh uselessly pushed into metal mouths just to drop down onto the floor into a pile of bloody, fibrous chunks.

As grim-dark as it sounds to us, probably the most horrifying thing is that the child undergoing this will probably be overjoyed and filled with pride and honour at all this. Same with those psykers that get sacrificed to maintain the Golden Throne. In-universe, those are not morbid things, they are in fact highly enviable fates, a honour that the rest of humanity will be greatly jealous of. And that is the most terrifying thing about 40k really. The Imperium itself. With its extreme level of fanaticism and brainwashing, the oppression and absolutely horrific standard of living for the vast majority of its population and the extreme brutality and indifference of its leaders the Imperium perverts and twists everything that could be considered 'good' about human society and embodies about everything that could make a place into a dystopia. The Imperium of Man is like the Nazis, ISIS, George Orwell's 1984 and North Korea had a child that grew up to be even worse than its parents. A place in which life itself often is a fate worse than death. And a regime like that is humanity's only hope. Now that is truly morbid and sick.

The way it seems in the fluff, the stock that Space Marines recruit from are a far cry from what we are as young teenagers. When we're playing with our little peebobs and still collecting action figures, these guys are probably spear-hunting giant spiked poisonous lizard-bear-wasps and murdering tribal warriors over a strip of dirt.
Now if you'll excuse me, I need to play with my little peebob and admire my action figures before I go spear-hunt something.
I mean, I'm going to sweep this little wasp's nest off the back porch, but it's still pretty dangerous.

I believe that fluff about Mad Dok Grotsnik offering his service, then replacing Ork Patient's Brains with an Explosive, which Grotsnik has the detonator to is pretty rough.
