As anyone ever codified anything like this? For any system?

I imagine it wouldn't do anything to enclosed vehicles with decent armour or very heavily armoured troops, but would cause a lot of ouch for anyone else...

He isn't the Omnissiah, he's just a very haughty boy.

I have never looked at Cities of Death, so there may be something special in there that cover this, but in general units embarked within a building that is destroyed (you can destroy buildings in 5e, remember) and explodes will take S4 hits, and anybody standing outside will take S3 hits if within d6" of the blast. Easily interpreted as being hit by flying rocks, that sort of thing.
