So ability's allow a unit to pile in and attack twice, does mean they can immediately pile in and attack again after the first pile in and attack or does it mean that an enemy unit can attack between the units pile in's?.

Which specific unit? It'll be hard to get an answer if we don't know how the rules are worded.

It's definitely case by case, as Kriswall notes. The wording for each ability should make it clear if the pile in and attack are immediate/back to back or just twice but still alternating with the opponent.

(well, maybe clear is an overstatement, but after looking through all of the warscrolls it does seem like GW took care to specify when a double pile in + attack could occur immediately)

Immediate/back to back:

Colossus of War: After a Bone Giant has attacked in the combat phase for the first time roll a dice. Add one to the result if a Necrotect from your army is within 18". On a 5 or more, that Bone Giant can immediately pile in and attack for a second time that turn.

Twice, but still alternating with the opponent:

Vanhel’s Danse Macabre: The undead are filled with magical energy that causes them to jerk forwards and attack with tireless, unnatural speed. Vanhel’s Danse Macabre has a casting value of 6. If successfully cast, pick a Skeleton, Mordant or Zombie unit within 18"; that unit can pile in and attack twice in your next combat phase.

The dark elves have a similar ability. I think the word Immediately is important, and like others pointed out each ability has its own wording that makes a big difference.

For Vanhel's Dance Macabre, that means that within a single combat phase a unit can be picked twice, rather than just once. This plays into the initiative system. If in a combat phase a unit with Dance Macabre was engaged with an enemy unit of clan rats, You'd pick your unit to attack, then the clan rats would attack, and then you could pick your same unit again to attack for a second time in the phase.

A good contrast is death frenzy, which allows a model who is slain to immediately pile in and attack. This is done outside the initiative system and happens instantly.
