I rolled something like 4 6s for invuln saves for a unit of flesh hounds once. They were S8 wounds, too, so it was critical for me to save them. My opponent actually checked my dice after that

Made constant 4+ FNP saves against a bloodthrister who was in combat with a Biker champion. A sorcerer was in the unit providing him with the biomancy saves.
B eat that bloodthrister to death with my power first over the course of three turns.

I can't remember the exact rolls but when my Riptide went toe to toe with a Bloodthirster in close combat for three turns without take a single wound. It was an epic fight right in the middle of the board.
Was a bit annoying for both of us having our heavy hitters tied up for most of the game though!

Mine was back in 6th Edition when a DA sergeant took out a Helldrake in 1 shot with his combi-plasma at short range, rolling two sixes and a 5 on one of the dice [other one missed] to hit, penetrate and cause an explodes result. Probably was about 1/90 chance or something.

A couple of months back I had a 10 man guardian squad charged by 5 Khorne Berserkers, I got him down to 3 models with overwatch, he killed all but one of my guardians. The lone guardian then proceeded to pass his leadership test with  . He survived locked in combat with them for 3 turns until the game ended and even managed to kill one.

In a game I played last week, I had an Apothecary who just would not die. He survived two rounds of shooting from a full unit of Fire Warriors (with Ethereal) and from a Riptide ( http://standwargaming.blogspot.co.uk/2015/10/battle-report-22-1850-pts-ravenwing-vs.html ).
In another game, I managed to save 18 wounds on my Terminators without a single fail ( http://standwargaming.blogspot.co.uk/2015/07/battle-report-11-1500-pts-white-scars.html )

Mine has to be when I was using a squadron of 3 FB piranhas against GK. They all had the automated repair system (was playing a friendly game).
Gks unloads some dakka into the group blowing up 2 and destroying the last ones fusion blaster. I jinked also. Next turn I roll to repair my wep, land a 6. move up to a Venerable dread, snapfire.. land a 6. managed to score a pen and for the vehicle damage chart.. I roll a 6! 4 6's in a row allowing my broken down piranha to 1shot a venerable dread. The GK player was dumbfounded.

When I rolled 5 attacks for my Warboss's power klaw on a weirdboy when playing  VS I think I rolled a  and 4 5's. Weirdboy had failed his force weapon attacks, and the Warboss killed him.

My luckiest die rolls happened in a tournament when I was running Dark Angels from the 6th ed. codex. Azrael managed to tank 22 wounds in a single round of combat without taking a single one. 2+ armor with FNP from an apothecary kept those Orks from hurting him!

A game I played recently I had a GK Librarian (warlord) with the Cuirass of Sacrifice... suffered 2 wounds and of course rolled snake eyes. Almost forgot about the FNP, but then rolled 2 sixes. It was quite the moment of truth
Although that whole game my Dreadknight didn't even make an appearance so that was fun... :|

I ran the dread mob at a local tournament. I kept pretty much everything under a KFF and by the end of turn three I had only lost a trukk, a few boyz, and 1-2 Killa Kanz. I was running the dread mob and the KFFs would not quite. They block all of the exorcist and melta shots the sisters could throw and the DA allies couldnt do anything. Also whenever I did pen a vehicle with all of my S8 shooting 75% of them were imobilizations... The player surrendered turn 3 because while he could have continued playing only one model could really hurt anything of mine.

There are one of two incidents of luckiest rolls for me:
1. A long time ago, I played oldcrons and was allied with a Tyranid player in 4000 pt Apocalypse match against a Tau and IG alliance. The tyranid player and I had brought stuff from within our respective codices, nothing particularly "Apocalyptic" (not even the Nightbringer or Deciever). The IG player however, had brought a Baneblade. First turn goes to the Tau and IG players and that Baneblade put out some frightening firepower (was the first time I had faced a baneblade or a super heavy of any sort for that matter) and frankly, I think I was saved from phasing out by turn 3 thanks to cover. Anyway, the nid player and I go and I make a desperate gamble popping a squad of three heavy destroyers out of cover in line for a clear shot on the baneblade's frontal armor. Shooting phase comes and these three quasi-automatons do my bidding and open fire on the baneblade. They all hit, then they all penetrate. I roll for damage and what do you know, I make the baneblade have a catastrophic/apocalyptic explosion which wipes out it and a quarter of the opponents' forces off the board. From there, the game was utterly and completely dominated by the nid player and I. With the overcosted codex I had. I felt pretty proud of that moment.
2. The possible competitor for the above: About two years ago, I made a grey knight army for a friend who loved Castellan Crowe and the Purifiers. However, when he had went up against any player in our group, he lost. It happened enough that he got pretty upset and asked me for pointers. We end up deciding on letting me play his army and he'd watch and hopefully learn. With his army, I took on a blood angels player. And frankly, the dice weren't all that kind to me but nice enough to bring the game to a Crowe vs. Sanguinor duel turn 6 (at which point, everything else on the board was dead, no other models, just crowe and the sanguinor). The Sanguinor was able to finally kill Crowe and the blood angel player was quite happy until I remembered that Crowe could take someone with him in death. Don't remember the dice roll but Crowe got it and took out the Sanguinor along with him. It was pretty damn epic.

This one game of Horus Heresy my Praetor and his boys were stuck in combat with a pair of Castellax robots and a babysitter. Opponent whiffs his rolls and I make like 9/10 hits with my master crafted paragon blade. I rolled a 6 round one. He fails his FNP and a Castellax is sliced half. I rolled two sixes round two and he failed his other FNPs. The baby sitter and the other big MC are decapitated. My one Praetor killed two MCs and a character that buffs them in two rounds of combat.

The one I'll always remember was in a game of Epic. I was Imperials vs. Eldar, and I had an Imperator Titan. In one turn, he dedicated all of his shooting (which included two titans of his own) against the Imperator. He did knock down all 12 of the void shields, but didn't do any actual damage (but, hey, all shield down). On my turn, I did my stuff (Imperator blew away one of his titans), and at the end I rolled the 12 dice to try to regenerate the shields. I forget if it was a 4+ or 5+ to regenerate, but I regenerated 11 of the shields.

I had a group of Guard vets in a building. My friend just summoned a bloodthirster (Cause he only knows how to summon. I will have another story) and was hellbent on killing them. He charged the squad and the squad did 2 wounds on overwatch. Then after the bloodthirster killed half the squad, the rest managed to plink off the other 3 wounds in CC.
Another time, my friend decided that using summoning in a Cities of Death game was a good idea. He summoned, throughout the course of the game, 8 squads of blood letters. All of the squads mishap'd into the exact same building and he rolled the "unit destroyed" result everytime.

I'm sure I've had many lucky rolls in all the years I've been playing this game, but the following is still one of my favorites.
In third edition I was playing Orks and my buddy Blood Angles. My mob of thirty boys was shot up and assaulted by 10 assault marines. Needless to say, by the time my Init. 2 came around I only had 3 slugga boys left. 9 attacks, 6 hits, 6 wounds. My buddy failed 4 saves. So I still lost combat, not surprising, but now it was LD check time. That I failed miserably. However, the old Mob size check stated that if an ork mob failed any LD/moral check they could take a mob size check. Roll two dice and if the number rolled was less than or equal to the boys present they pass. Well, I needed a three or less and rolled a 2. The boys hung on thus kept from getting swept thus keep the BA assault squad from consolidating into another unit of Boys. These three orks pretty much saved me the game as I was able to mount a counter offensive and turn the tide.
