Getting into some narrative play with the Heresy.

So we have models/rules/lore for Centurions and Praetors, and its pretty clear where they sit in the hierarchy.

What are people doing (if anything) for Lieutenants and Lieutenant Commanders?

I can see how a a few of the Consuls could make for Lieutenants - holding a specific role before advancement into a more wholistic warfare position of Centurion. Or just using a cheaper Centurion in lower point games?

Same for Lieutenant Commanders. Practicing a deep specialisation before movement into the upper command echelons. Or just a cheaper Praetor?

Centurion is just the Principia Belicosa designation for an officer of lower rank than a Praetor, which itself is a very broad scale.
So for example a Centurion could be anything from a Lieutenant to a Company Captain, while a Praetor could be anything from a Company Captain to Legion Master (basically second in command to the Primarch).
As well as this, within the narrative framework of the Heresy saga, characters like Aeonid Thiel who was just a Sergeant could be represented by the Praetor rules as he led his own personal strike force of veteran Ultramarines and wielded some of Guilliman's private armoury stash.

In terms of what you're talking about, I would have said that the bog standard Centurion is analogues to a Lieutenant. An officer, holds a level of command, but nothing specialised

As for a Lt. Commander, the Delegatus looks to be on the mark. Deeper into the tactical command side of warfare, is given charge of missions in the praetors/chapter masters absence and has seemingly higher authority to other centurions/consuls, (in theory sitting 2nd only to the praetor himself in the chain of command,) when it came to their specific mission. Some of the specific legion consuls might offer other options for what you're looking for though.

The available options in the Libers obviously aren't as granular as the what the command structure is presented as in the lore, so you're saddled with a relative lack of choice.

It's also important to remember that Consuls themselves will have ranks beyond their titles and that will vary from Legion to Legion.
A Librarian in the Thousand Sons will have a high rank such as Captain or even Chapter Master and a Siege Breaker in the Iron Warriors would be much the same.

Where is the “standard” legion structure laid out?
