It's from a Forgeworld Bulletin from this year, so I suspect it's a paint job on one of their standard pieces.

i thought about painting up a helm like that before
if you cant get a mk II head you take a cone head... cut it..
and voila you have a bad ass mk II head

there is also the scull helm from the warriors of chaos works too

You guys are good. I never would have thought that was a simple paintjob.
