I am just curious how one would sculpt a Bolter holster (with green stuff), and if one did where they would put it on a SM or CSM.

You mean for a full sized Bolter? Ive used a couple things for models like Standard or Icon bearers etc. Instead of a holster Ive found its much easier to just make a sling. a Thin piece of green stuff so you can drape it over a shoulder or as a belt.

Or if your just going for Fluff (an assault squad with a backup arsenal etc) and the squad / model in question has a transport or body guard its fairly easy to make a weapon rack on an out of the way part of Rhino or have a peon carry it (Most people wont object if you have a separate model who's only purpose is to carry the weapon for a commander no attacks wounds etc ((ie: Commander Azrael or the Dark Angels has his Helm bearer)))

"All right, boyz, 'ere's da plan: Win. An' if we lose, it's your fault... 'cause you didn't follow da plan."

Firstly, thanks slings seem like a much better idea.

But wouldn't you attach the sling to the bolter it-self? I was thinking something along the lines of the tyrannic war vets where the sling is attached to the bolter.

For slings, you can try something like what I have done on my storm bolter conversion below:

Basically it is a strip of plastic sheet cut to size and then glued on.  In order to make shaping it a little easier, you can submerge it in hot water for a few seconds first.

Hope that helps some.
