Most of the numbers I have come from a few local stores, where about 90% of 40k sales are Eldar, Space Marine or IG boxes. Maybe it's different in your area, but the box you're referring to simply doesn't sell. And out of all the people who play at my FLGS, I'm the only one with any converted models.
I say vocal minority because for every single kitbasher out there (and your definition is in fact correct in my opinion) there are ten kids who'd rather just buy a Space Marine tac squad and be done with it. At your store, yes, the Commander box sells - but how many Space Marine tac squads do you sell? I'd be willing to bet a lot more. And if not just Marines, then consider all the Guardians, Cadians, Ork Boyz, Necron Warriors, etc. that are sold. Consider the ratio and tell me if you honestly think it favors the kitbashers and modelers.

As far as I'm aware, the current (plastic) monolith is the first and only one GW have produced. It was released with the current codex... before that, the Monolith didn't exist in the game.
That's what I thought...

As much as i dread assembling & transporting a metal model myself I don't think it'll happen soon. They only update a few models & units per army for each release wave. Units & models that sell in multiples per army have priority on their release schedule. Also named characters usually don't have alot of options, they're the category who benefit the least from the options provided by sprues, while rank & file can really use the benefits of extra options to avoid looking like an army of clones. Feel free to call me pessimist but i also magine them having a few pallets worth of superglue and greenstuff in stock
About scenery i feel it should always be in seperate boxes, suppose i get a character with his feet embedded in some scenery and i just made a table with a theme/color that doesn't fit the premade scenery? Then it's back to butchering the model or redoing the scenery.

I thibk a good but basic idea would be to make a box called the special character box, it would cost say 30 british squids and you could make about 5 any of the chosen armies heroes that way youd have to keep buying the box and GW would profit?

another part of the problem for GW would be people who would magnetize the Special character box and get 5 special characters for 20$ instead of 5 for 5*12 or 60$. I think that was part of the decision with the manticore/deathstrike kit being made the way it was as it is harder to magnetize.
