We are in the 5th Edition for Warhammer 40,000.

No, it doesn't make sense that the Demolisher's shell scattered back onto itself, and blew up the tank which fired it...but if we were writing in as an AAR, we'd explain that as the shell blowing up prematurely inside the tank. That Rhino you missed from point blank range? If this was reality it's careening back and forth, you're trying to avoid getting shot yourself, and maybe not taking the time to aim shots. Your gun may have jammed, or you may have to reload, and as such you miss the time frame in which you could have shot your target.

With a d6 system, and the streamlined rules you have to rely on abstractions. Part of the fun of writing battlereports and AARs is justifying all the wild crazy things that happened.
