Maybe not 40K, but definitely from the source material:

"Muad'Dib."

His name is a killing word, if you believe David Lynch.

What harm can it do to find out? It's a question that left bruises down the centuries, even more than "It can't hurt if I only take one" and "It's all right if you only do it standing up." Terry Pratchett, Making Money

"Can a magician kill a man by magic?" Lord Wellington asked Strange. Strange frowned. He seemed to dislike the question. "I suppose a magician might," he admitted, "but a gentleman never could." Susanna Clarke Jonathan Strange & Mr. Norrell

That's Horus line is awesome, and is probably responsible for the most human deaths, but I found an Eldar scientist note in my Ork Codex that says the Orks were created by "something older than us to fight the great shadow of the galaxy".

Since the Tyranid are a recent threat, I think they were talking about the Necrons.

So, I think this means the lie from the Deciever's lie (shrouded in mystery though it may be) is the most destructive sentence in the galaxy.

I also submit a theory for what that lie was: "I offer you freedom from death and desire. You will be content and immortal." And he did that. He made them immortal by killing them, and changed their desires to suit his needs.

Special Note: I didn't even consider the C'Tan as candidates, so thank you for bringing them up.

The start of a book so open to interpritation it has caused countless deaths, wars and pogroms throught out history and is pivotal in the whole 40k Mythos.

The Old Ones created races like the Eldar, Jokaero (monkeys!) and Krork to fight the Necrons.

Maybe the words "Death" and "Star" next to each other in a sentence will result in..death..

Shotgun also certainly spells death for anything.

"they will be my spacemarines, and they shall know no fear"; I'd guess the creation of the Space MArines led to a ton of deaths (countless more provided the Emperor said this in reference to creating the primarchs as well), though i suppose since he said this POST marine-making, it might not really count...

If we expand it to include other sci-fi realities, then we'll be here for a very long time.

Doctor Who has been destroyed.. I miss David Tennant, they never should've killed him off. But, career advancement called. The current Doctor Who series appeals greatly to a much younger audience than even myself.. but in doing so they've removed the feel of it.

Continue on.

It was said in passing earlier, but never put forth as the word that is probably responsible for the most deaths in real life as well as any fantasy or sci fi you can find:

Usually preceded by:

That's interesting, poke it and see what happens

Eye of Terra.

"That is no longer canon"

or

"There is a new codex coming out"

These two phrases have, by turns, caused the eradication of entire civilizations, thought processes and the needless 'filling up' of thousands of pairs of underware across the universe.
