Haha, I can't comment on 40k fiction but according to the IG codex commissars don't execute their own and by extension if the commissar is broken then unit breaks as well.

I'm sure there's a moment where two commissars glance at each other.

Commissars never run away. If a unit with a Commissar flees, the Commissar is clearly chasing the cowards and trying to embolden them to return to the fight. In the highly unlikely scenario that a single Commissar flees (because the rest of his unit has been killed), then said Commissar is withdrawing from his untenable position in the field in order to link up with a more worthwhile unit of soldiers, who won't profane against the Emperor by dying before the job is done.

But in all seriousness, if I saw the officer charged with keeping me in line and fighting running and screaming I would assume that the trouble was far too much for us to handle and would follow him.

next IG codex needs to have a rule that says any time a commissar flees for any reason all IG units with LoS to him must immediatly take a morale test or flee as well.

Commissars are trained for years, decades even, to be loyal to the end and have unwavering faith in the emperor's cause. Men with such diehard belief have a hard time giving up(see wars involving religious extremists like crusades). If they are breaking and running, there most likely an order of retreat in play somewhere. A trial will most likely be held if the war is survived.
