Malcador the sigillite was distantly related to the emperor acording to the horus heresy collected visions book, he sat to the right of the emperor on his war council and his origins were unknown to all except the emperor hes abit of a mysterious character really

How did Mal survive meeting the Emperor considering how powerful Mal was and the big E was going around killing most of them because they got the blame for old night. (when warp travel became impossible)

Everyday people blamed them for old night and at the start of the great crusade they came across worlds suffering under xenos and psyker rule and hence - suffer not the heritic, witch and the unclean to live - but your right though, the big E knew the real reason why.
When was it said he was the Emperor? I know it has been hinted at but I'm going to wait till after the seige of Terra (in HH) to make up my mind if he is or not.

It is theorized that the Emperor created Malcador to be sort of a second body. A part of his conciousness was split off and is inhabiting Malcador.
Which would make sense. When the Emperor was returned to the Golden Throne after the battle, Malcador poured his energy into the Emperor. Which gave him a little time to speak with his subjects and instruct them on hooking him up to the Throne, and also to found the Inquisition.

It is up there with "The Outsider is the Tyranid Hive Mind" in terms of unlikely fan theories.

The Inquistion sign was originally Malcador's personal sigil, in the audio book Oath of Moment, Garro has it carved into his armour as the only symbol of recognition on his otherwise grey, unadorned armour. I think it's safe to assume that after Malcador's death and with the rise of the proto-Inquisition they adopted the sigil wholesale and it's caught on big time.
