The highest point game I've played was 650 in GW store event, my usual games are no more than 500 as my buddy and I are still getting used to the rules.
I do find myself INCREDIBLY limited for effective units at this level as a DE player though, and want to my next games up to 750 so as not to put too much on the table that will end up with us referring to the rulebook even more than we already do now. I can't wait to get to 1000 points though just for what I think will be that "real feel"

I'd say 1600 for fantasy and 1500 for 40k

Okay. 500 point combat patrol. This is one of the best, Always Full tournaments with a long list of alternates that I run once every year for the past 14 years. It comes down to the thought of having fun.
There is also the Kill teams approach as well.

Also, how does one apply to be a member of the Ultramodrines? Are harsh trials involved, ones that would test my faith as a wargamer and resolve as a geek?

750 is the minimum I'd play for 40k. While Combat Patrol is nice, it doesn't lead to a lot of variety and the rules are outdated.

Not really as in my case the Combat patrol rules that I use have been implemented for 6th ED. Variety? Eh there is enough to make things every interesting if you use your mind to do so.
