Hmm...I guess that could be subjective. Here is my most ruined model. See the text following the pic for my explanation of it being ruined.
It was ruined during my Noob stages about 6 or 7 years ago. At the time I figured Grey Knights could fall to chaos like other marines. I didn't know much regarding fluff and I thought a Chaos Grey Knight would look cool.
Needless to say, I eventually pulled the thing apart, ditched the spikes, and stripped the paint off. I repainted it in proper Grey knight Colours, so I hope I have been redeemed.
What a way to start the thread huh?

I made most of my mistakes as a Noob too.

I got one in the post, someone had undercoated a terminator with some sort of terrible spray paint, it had mostly melted it, looked like the squad had just come out of a bog.

Probably would have made good Nurgle terminators though.
I feel sorry for the guy I sold my grey knights too. He didn't know they were tainted. I bet they always failed leadership rolls...

2 years ago I tried to paint my first terminator. The first mistake was to not shake the spray can, then i tried to strip the paint off it with acetone free nail polish and the mini kinda melted, now he's really ugly.

I have done that as well. They are serious when they say shake the can vigorously for 3 to 4 minutes aren't they?

First army I bought in the store. I asked for a clear finish, and the guy handed me " 'Ardcoat"
First model was my centerpiece= Pewter Venerable dreadnought. Assembling and painting took me 8 hours, and when I was done, I sprayed the 'ardcoat on it, and the whole thing was covered in goupy grey primer. Yea.

My most ruined model has only come to light recently.
So picture the scene, the previous orcs and goblins release:
I had a horrible mismatch army made up of spare parts, scrounged miniatures, and old ebay pieces. It was a shambles. Anyway, I thought, what with the then new rules, I'd get some Spider Riders. So I bought some, and it then occurred to me that I couldn't be bothered to do anything with them after I half built them. Now the real fail in this story, was that I mindlessly cut up the spider riders boss arachnid. I don't know what for exactly now, but I had a reason a the time I think.
Now with the Arachnarok coming out, and my gobbo passion reigniting, I find myself with 9 spiders to 10 riders...
Oh! And the feathers on most of the gobbos were cannibalized for other projects!

Worst experience with painting occurred here...no pictures, but it was a mess..I did eventually rectify the problem with a very long soak in simple green for the affected minis.

I once planned to repaint my old LRC, and had bought this special plastic paint-stripper from a well known parts company. The stripper was like some sort of paste and the instructions said to cover the model in the paste and leave to dry in a plastic bag...
...*48 hours later*...
...I open the bag and the LRC is a mess. Hull plates have been melted and bent, and there are burn marks around other areas. Worst product I have ever bought! ¬¬

I'm not really seeing this as a Showcase topic, particularly since so far it's largely just people talking about their models rather than posting pics.
Moving to general P&M.

Ultramarine Master with Gauntlets of Macragge

I got this salvaged "predator" from eBay once. It had a pretty average predator chassis, albeit very poorly constructed and covered in drill holes, glue damage, knife marks, and all kinds of gross paint clots. It had a deformed and glue-encrusted Marine sitting in the cupola. The icing on this ugly cake was the turret - two bases glued together with a screw through them, and a predator autocannon awkwardly sticking up at a 75 degree angle. A few months later I went to it with a knife and spare parts. It took me somewhere around 6 hours to clean it up and rip it to pieces. It later got repainted black and made into a death company rhino, and at this point I think it's actually my Orks' looted wagon.
EDIT: Here's one more. I've been painting on commission for a while, and I was doing some blends on some Sanguinary Guard. They were looking awesome, so I let the blended swords dry. When I checked them later, they looked like this:
Tried three more times, no dice. Fifth time will be the charm.
