Hey,
I have made a few Ork trikes and and Ork sidecars and buggies...etc. Mine arent the best but not the worst.
Spoiler:

2 easiest ways I have found are;
1.cutting the back off a ork bike and adding an axle with 2 wheels,IE 2 wheels glued to a straw or axle looking bit.
2. cutting a deffkopta blade arm off the top and adding a wheel to the part the blade would attach to. and then stick a wheel on the front.
I oversimplify a bit.

Just finding an extra wheel somewhere, and then just manually building the rest is probably the best (and orkiest) way to do it.
