It's the Dok's Tools that give FNP, check his wargear, and on page 38 in the description for the Pain boyz wargear.

If a unit has a model with dok tools, the entire unit gains a 5+ FNP. Mad Dok Grotznik has dok tools as part of his wargear.

7 Ork facts people always get wrong:
Ragnar did not win against Thrakka, but suffered two crushing defeats within a few days of each other.
A lasgun is powerful enough to sever an ork's appendage or head in a single, well aimed shot.
Orks meks have a better understanding of electrics and mechanics than most Tech Priests.
Orks actually do not think that purple makes them harder to see. The joke was made canon by Alex Stewart's Caphias Cain books.
Gharkull Blackfang did not even come close to killing the emperor.
Orks can be corrupted by chaos, but few of them have any interest in what chaos offers.
Orks do not have the power of believe. |

I think your rulebook is an edition out of date... Default FnP in 6th is 5+ (as per page 35 of the Rulebook), and most FAQ's have been updated to reflect this. Are you meaning it says 4+ in the Ork Codex?

It doesn't, I just checked both the Codex and the BRB. He might be confusing the Dok's Tools description with the 'Urty Syringe on page 38 (PainBoyz) of the Ork Codex. Dok's Tools confer FNP to his unit. 'Urty Syringe is a Poisoned Weapon that always wounds on a 4+. That, or he's confusing the description of FNP which says 4 or less you take the wound as normal.
Or he could have an old rulebook... mine's in storage so I can't look it up.

On page 59 of the Ork Codex Mad Dok Grotsnik. In his wargear he has Dok's Tools
goto p38 for Painboy under wargear - "Dok's Tools: A Painboy is an expert at repairing the sturdy Ork physique using a variety of mean-looking tools. He confers the Feel No Pain ability to his unit."
Mad Dok might be one scalpel short, but he still confers FNP to the unit he is attached. FNP is a 5+ extra roll after save.

Yeah I was stuck in 5th edition rules. Thanks for correcting me. I've never ran a unit with fnp so I was going off of what I remembered.
