I would just like to add my two cents to this topic:
In reference to WH40K, the game does not even work unless you have house rules/proposed rules. Literally, the game does not work. There are numerous gray areas where players will just have to agree on an intrepretation and then play it as is. Otherwise, who knows how it is actually supposed to work. I voted for "other".

I have played a bit of KillZone, which is a cleaned-up thought-out version of Killteam. Fun game that expands on a good idea from the GW Mission Book.

As for regular 40K, Kurce is right - dozens of small adjustments just to keep the game rolling... I wouldn't say we are making up new rules, but ya gotta make a call, keep it fun and just keep rolling the dice.

With some of my friends we will just make up some new rules before the game to make a scenario of sorts or a different game type... one of our favorites is that any unit fully wiped out is returned to the reserves.

Incidentally I only play with FAQ's and such, but don't mind house rules as they can make the game very fun! In fact one of my favorite (even though I don't play them) is that at the Chicago battle bunker you can use the stormraven in any marine army. Just goes to show that GW employees like to have some fun between trying to shove WD down your throat. Tee hee

We have a house rule for flyers at my local GW.
Move at least 36", but other than that, no limts.
Fire all weapons
Cannot capture objectives
You need sixes to hit the flyer
Flyers can't ram (unfortunately, because I would love to ram a titan).

Within reason, we have our own way to create special characters as well as allow character death, campaign momentum, and as well as making it easier for reserves to come in.

There are a few house rules that my friends and I use when playing games. If we are at a GW we ONLY use the rules as written in the book and official FAQs. At home, we have use these rules where permitted. Of course we talk it over before the game starts because sometimes we prefer things to stay and others to go away.
-Only play Dawn of War setup in a campaign game where it is appropriate for the scenario. (We all hate the setup and feel it gives too big an advantage to first turn for all our armies).
-Instead of Kill Points we use old 4th edition victory points. (One thing I LOVED about 4th edition, call me crazy).
-Objectives are worth 200-300 Victory points. We feel that even if someone would win by holding more objectives, if their whole army was destroyed besides those three or so units, then they would lose it in another minute or two anyway. (for cinematic purposes). We don't always use this rule though.
-NEVER use seize the initiative. (This was actually my doing, as I almost always end up stealing it and screwing my opponents over because of it. I have won games at home and at the GW because of this one roll and I always felt that it was a cheap way to win, and a stupid way to lose).
Keep in mind that my friends and I play highly themed and cinematic armies as opposed to straight up 'win at all costs armies.'
We play:
Me- Deathwing/Ravenwing (2000 pts), sometimes pure Deathwing (1500pts.), Orks (1500 pts.)
Opponent- Space Wolves (heavy with terminators and long fangs, and Ragnar Blackmane)
Opponent- Home brewed Chaos Space Marines codex (similar to ye olde Lost and the Damned Codex). Or he uses the Space Wolves codex as a counts-as Chaos Codex. Beginning a new IG army.

It matters how much beer is at hand.. For local games we play pretty much by the rules 100%... But Beerhammer changes the game just a little. For example I have used lash to send my opponents off of buildings, space wofl terminators have used the overpowered elbow drop on more then one ocasion, and bugs have been destroyed by Mephistons psyker fly swatter a few times...
Remember al'ls fair in love and war, and there is no love in 40k.

Me and my friend do when we are playing a ffa since the rules are made for 1v1s. New objective grabbing rules, shooting in assault rules and a couple other slight variation to make things realistic for a ffa.

We use house rules for missions/campaigns since we like to make up our own, and we also change some rules that may seem heavily imbalanced. We do not change them though to the extent that rule is different, but rather maybe make exceptions to ridiculous rules and we also make up house rules where there are gray areas. A lot of times our house rules on gray areas are decided by a dice roll.

It depends on who I play with. I have friends who I generally follow the rules with, and also have friend who utterly destroys the rules to the point where its hardly even the same game. I have fun playing with both.

We have a rule called 'bullets dont stop in mid air', which is basicly saying that if a model in a unit is in firing range, all models are concidered to also be in range, so the poor ork boy on page 17 of the brb would be able to fire.

been playing for umpteen years and we keep a little of things we liked from previous rules editions. We often use Dark Millenium strategy cards and mission cards. Sometimes also still use victory points and (very) dangerous terrain. Remember the carnivore lair from white dwarf or something? It's a terrain piece with a nasty inhabitant, and sometimes models that go into it don't come out.

We don't use the ally rules in fantasy either, they would only be applicable in a campaign I think. For a pick up game amongst friends if the armies can ally they do it for a one time beneficial reason.
