I know that Iron Hands don't have a Chapter Master per se and instead have a council where (if I remember right) there are a few Dreads on it.
But is there any SM chapter mentioned where the actual Chapter Master is indeed a Dreadnought?

I think the closest might be Space Wolves with Bjorn the fell handed.
What happened between him becoming great wold (chapter master) and becoming a dreadnought isn't explained all too well. There is a chance that for a short period of time he was both. He is an HQ choice dreadnought, and in lore he was a chapter master. In game terms he probably as close as you will get.

As I recall one of the Blood Angel Succesors was lead by a Dreadnought before he was destroyed.
It was Chapter Master Daggan of the Blood Swords.

Not chapter master but even the great wolf takes his advice seriously and weighs it carefully.
He is respected on a level pretty close to chapter master.

Dyanthenos was interred to a dreadnaught throughout this entire period.
