Very quick question.
In combat, you go in Initiative Order. This is known.
When removing casualties in combat, just as with shooting, you remove the models closest to the attacking unit. This is also known.
BUT.....I can't for the life of me remember if a unit that was in BTB at the beginning of the combat round still counts as BTB when it's their turn to fight, even when the enemy unit has had enough casualties removed to isolate them. I swear that was true in 5th Edition (the last time I regularly played 40k), and I keep forgetting to check it when I've got my rulebook to hand (might invest in the digital one!).
I suspect it's not, and it's therefore now possible to lock my own units out of combat through heavy enemy casualties, but just need to make sure!

No you don't count as being in B2B. You just go by what the models physically are doing when you check

You do a pile in at each initiative step. So somebody should be in B2B contact when it is there turn to attack. So real crazy crap would need to go down for nobody to be in B2B without eliminating the opposing unit.

I've got St Curia's Autopurger going off at I10, against Orks that'll be I1 (mmm....Sicarian!) - so there's every chance I'll cause so many casualties, I can't help by leave some of my dudes out in the cold!

Buddy of mine bumped some lightning claw termies into a blob of boyz once. On my initiative step we bumped in and had no eligible CC left to resolve except me failing a morale check. This is the kind of stuff that has to happen. In fact we aren't even locked in combat anymore if I had passed.
