Nothing comes close to the insanity and pure metal soundtrack. Little 7 year old me was blown away by it in 1992 on the Sega Genesis (Mega Drive to us Europeans!)

Dead Cells is one of my favorites of all time, and it's a pretty recent one.

True, sorry if I was vague. Actually any of those. Any game of the late 80s/early 90s using a side-scrolling platform (be it a beat 'em up, shoot 'em up or adventure).

Konami used to rule on side scrollers. Contra, Castlevania, and the like. My favorites were Symphony of the Night and Cybernator. Damn near wore out the cartridge for the latter.
For arcade games I remember feeding lots of quarters into Rastlan, AvP, Golden Axe, and Altered Beast.
In terms of bullet hell, I think Ikaruga still stands at the top, though while its far from the best, I've still got a soft spot for Einhander.
In the Beat'em Up sub genre, I don't think anything has really topped River City Rampage, though Turtles in Time tries really really hard. I need to give River City Girls a playthrough on that note.
As far as beat'em'ups go, I really loved the x-men arcade game with the absolutely terrible voice acting.

I will give a shout out to a rare but awesome multi-scroller shooter on the Megadrive, and its a crime against humanity that it isn't more well known...
...this was the game that drove me to obession to learn how to program for the Megadrive, and what a machine it is! If anyone tells you the SNES was better or vice versa, don't listen to them! Both are incredible machines that ultimately boil down to producing two different styles of presentation and in very different ways. Never in the history of gaming has two machines gone head-to-head and took the world by storm.
Now, prepare yourself for this one as a very talented programmer has showcased the true potential of the Megadrive without support chips...
