In the new Chaos book it says for every 5 models 1 can upgrade the combi-bolter to a heavy flamer or reaper autocannon for whatever points. Does this mean a player has to actually purchase 5 models for the unit to get one of those weapons, rather than the basic unit of 2 Termies/1 Termie Champ purchasing an upgraded weapon?

That's precisely what it means - you can't have a heavy weapon if the squad doesn't number at least 5 terminators.

And for the next question - no, you can't get up to five models by taking four and deciding to have your CSM termie lord join the unit on deployment.

What about if you buy a squad of 4 Terminators and buy a dedicated Land Raider. It counts as a model in the unit does it not? Because it is purchased as part of the unit right?

No, it is a separate unit, like all vehicles.
