Chapterhouse Studios offers a conversion kit, making it a bit bigger. Just greenstuffing a sac to a Carnifex will not do sizewise.

If you put him on the correct size base (trygon/dreadknight/flyer base), and bulk him up a bit, and make him a bit taller - I think most folks might be fine with it.
Depending on your skill, I would try:
- aluminium foil for shape and size, covered with clay or green stuff
- a golfball covered with plaster then fill in the gaps.
- just a big lump of clay, then texture it.
The chapterhouse kit is not bad either, but some people do not care for it.

Surely it depends on where you stick the sac. You could easily hollow out the upper parts of the 'fex and have a big thing coming out of its back. An example would be that frog that keps its babies under the skin of its back and they pop out when they are a certain size. Horrible, but possible!

I thought they had to stop selling that one, but now that I think about it, the last court stuff I saw in CHS vs. GW was over shoulder pads and space marines.
