tell me your awe/tear/laugh inspiring tales of self defeat on the table top. Gets hots, DS mishaps, bad blast scatters, and so on.
One of mine was during one of my most recent games. I like to take lots of plasma in my DA. I also run divination librarians. Typically I put them in things like 4x plasma gun + apothecary command squads for ignore cover + reroll to hit fun. This time I decided to take a 4x plasma cannon dev squad with a one of my ignores cover librarians. After inducing a bit of rage from my opponent at the lack of gets hot and great scatter rolls I managed to perils on turn 3, and have the librarian (my warlord) take the whole squad with him into the warp.

I have a Chaos Lieutenant on bike that has splatted himself across more trees (died to dangerous terrain checks) than I can count, but the most devastating and game changing one was allowing my Daemon Prince to get force weaponed to death by a librarian simply because I completely brain farted to allocate any of the DPs attacks against it (the DP goes first, allows no saves of any kind, and hits on 3+ and wounds on 2+.

Picture this, I have a daemon army focused around my Keeper of Secrets and Be'lakor, I equip my keeper with soulstealer and greater rewards (this game I rolled re-rolls on my invunerability saves) I'm facing more than 4 squads of Sisters of battle and 3 long range artillary, my opponent knew how much of a threat the soulstealer is so he begins to focus the keeper from turn 1, so he begins barraging my keeper with everything he has, be'lakor is keeping him alive with invisibility and this continues untill turn 3, my keeper has survived thus far on 2 wounds left after losing 3 wounds to ALOT of attacks, and now is right next to several squads of Sisters, this turn if I get my keeper of secrets into combat he will regain all his wounds thanks to the soulstealer and decimate the enemy, I begin my shooting phase... so close to assaulting and essentially winning the game... then I roll a 3 on the warpstorm table, the keeper literally explodes thanks to instability and everything goes to gak and I get tabled 2 turns after  it was a hilariously fun game and taught me how fickle the gods of chaos can be

Last turn of a close game with both me and my opponent just about equal in victory points but with me with a slight lead. I line up my warlord Big Mek's Shokk Attack Gun at a weakened squad of Chaos Space Marines. I roll for strength and instead get the result where I am teleported into combat where the Chaos Space Marines proceed to punch my poor ork to death, losing me the battle.

I think the worst things ever got for me was when I learned why people call them "suicide crisis suits." >_< Mostly, the embarrassing runs of bad luck have happened to other people I've played (both of them Dark Angels).
In my very first 500-point game, for instance, my opponent rolled a horrible Deep Strike mishap and teleported an entire squad of Terminators into a wall. Roughly half his list, gone to one die roll.
Then in a 750-point game, the Dark Angels' motorcycles were trying to flank my gunline while a tac squad fired on it. But they got horrible reserve rolls, came in at a weird angle, and then got, like, snake-eyes twice when rolling charge distance.
I figure they got lost after they took that wrong turn at Albuquerque, and by the time they were in sight of my Kroot their bikes had run out of gas.

Back on 5th, when stealth suits could take two drones each, and drones crashed down when their controllers died.
A team of 3 stealths, 6 drones make a thrust move into dangerus terrain. all drones pass, all suits fail.
Suits them continue to fail every armor save.
And down goes the entire squad.

I think the only real example of this I have, and it's probably not even a good one, was during a doubles event.
I had Daemons, my partner SM, playing against a BA/Eldar (before new codex Eldar) army. Tied on objectives, so our opponent charged their WK into my Horrors who were holding an objective on turn four. I hold on in the combat, taking no casualties as I had a 3++ save for that turn. Turn five rolls around and my partner charges his terminators into the combat to hopefully do some wounds to the WK. He did one wound, and took two in return. Terminators are of course fine for morale, but the Horrors who have taken NO damage thus far and are still technically holding onto an objective? Nah, they decide to roll double 6's and go back to party in the Warp. Windriders proceed to swoop in (swooping is bad...) and take the objective as they do and net our opponents a narrow win as the game ended there. Silly instability tests...

I say this every time one of these discussions comes up because it's such a pathetic/funny story.
My IG versus a Green Tide.
My gunline had failed to stop the Orks from making it to the walls of my Fortress and so I fired the Deathstrike Missile Danger-close. It scattered back onto my Fortress and destroyed 3 sections which killed over 40 Veterans, an Inquisitor and his retinue and my last Tarantula sentry gun. After that, the Orks just washed over the remnants of the fortress and mopped up my survivors.

Nids vs Tau.
Went for a podded Swarmlord, Trygon, and some other supporting deepstrikers. Everything got gunned down except the Trygon and SL which were near as expected. Tau cornered by the Trygon, Swarm moving up and the SL. The SL on his last wound an inch from combat with the swarm going to take the overwatch, the SL perrils and dies
Not the first time, not the last.

Magnus the Red and his terminator bodyguard,in heroic combat with a Wolf Lord and thunderwolf bodyguard.
(Using rules for Lorgar).
Magnus weathered the charge and takes a wound from the wolf Lord in the opening charge, but holds due to invisibility.
Tries to cast invisibility again, and Magnus and half his terminators dissapear into the warp.
Historically accurate wargaming.

Well, if I ever finish a game without at least one of my renegades being killed by a friendly template then that game was just boring. I blow up my own renegades because it's funny, and I do it regularly. It's not really that special, but my friends and I always make a point of it.
A friend of mine who is not on Dakka also has Plague Marines only ever die to Gets Hot, never enemy fire. Every. Single. Game. Usually on the first shot to boot.

In the first of my two total games of 7th, my Primarus Pskyer rolled a perils turn one, and summoned a herald of whatever. Turn two, another perils, exploded, took out my CCS warlord. By the time we called it, my opponent (demons) had more models than he started the game with, and I was pretty much wiped out.

Had Kairos experience a Warpstorm roll inflicting the dreaded 3d6 daemonic instability (1 in 3 characters) after rerolling -1 Invuln saves and burning his Tome of Fate on a Grimoire roll. Proceeded to roll 3 6's under the effect of Shadow in the Warp (-3 Ld to Psykers) causing 12 non-save wounds. My opponent got First Blood, Slay the Warlord and a Victory Point before taking a turn. Was definitely a fickle dice day.

My Ork Big Mek with a shokk attack gun has a hilarious habit of warping himself into close combat with horrible things. He's been eaten by lictors, squished by riptides, stomped by dreadnaughts, even trampled by rough riders. He did redeem himself last time by warping into an IG veteran squad and surviving long enough to tie the unit up for 3 turns.

Daemons versus Space Wolves. DP had killed almost all, but two TWC (a regular guy and a Sergeant), wounds, should have killed the last two TWC, but he makes three 6+ FNPs. My DP then rolls Instability (since I did no wounds), fails his LD by 5, and gets sucked back into the warp. He then brings in his Bloodclaws and proceeds to steam roll the rest of my army.

My Chimeras will take any excuse to break themselves. The worst was a 5E game. Last turn, have 3 Chimeras loaded with scoring Troops within 12" of the objective...and all 3 immobilize themselves trying to get there...

Was tempting, but I sucked it up and managed to only lose by a handful of victory points. I got a story and the game was fun in the end.

5th edition Apoc game. I drop a sternguard squad with meltas and combi-meltas next to a Baneblade. I blow up the baneblade and then lose all of the sternguard except the sergeant in the resulting apocalyptic explosion. That sternguard sergeant with his meltabomb goes on to assault and destroy two different Leman Russ tanks in subsequent rounds until the last one blows up and kills him.

My inquisitor always seems to luck out when casting vortex of doom Automatically Appended Next Post: The best one must have been marbo + demo charges. This was always glorious and interesting.

Game last year with my space wolf friend.
I manage to get first turn and he fails to sieze.
Tzeentch Sorc at 270 points wounds himself with scrolls of magnus at the start of turn one.
First psychcic test trying to cast endurance I Perils, roll a 1, and manage to kill him and 2/3 oblits, with the 1 remaining only having one wound. Slay the warlord, first blood, both scored for him without even having his first turn start yet.
I pull up my sleeves and say okay my 560 point tzeentch terminator unit can maybe still pull this off. Turn 2 comes, I roll for reserves and they come in, scatter 12 off the board, and then roll a 1, terminators now dead without even seeing the board.
I normally don't concede until the game is over, but after that I conceded.
Something about a few marine units with cultists vs an 1850 space wolf army that is relatively unscathed not being fun.

Quite a few instances. Wall of text but these 7 games stand out as pure laughter and fun while playing 40k. The worst of the Worst
1. Celestine and 10 seraphim tried to deepstrike and got lost to the warp with a mishap roll of 1(6th edition)
2. Storm raven carrying 5 terminators and a dreadnought, crashed and burned, killed every one of them and the Dread(7th edition)
3. 5 Terminators and Lysander declared a charge, got shot to pieces against Dark Eldar poison weapons. Lysander was the sole survivor with 2 wounds left.(6th edition)
4. Ravenwing black knights with Sammael got held up in combat the entire game by a unit of Sisters of battle led by a cannoness.(7th edition)
5. My favorite was while playing as orks(5th edition). full unit of lootas lined up to lay waste to the enemy. My dakka jet came onto the field. Opponent used Puppet master during his turn on my Lootas, turn them around, and shot my Dakka jet to pieces. They shot 45 loota shots at it.
6. Playing as tyranids I played a huge Nidzilla(5th edition), clumped up and going up the field. Space wolf opponent used Jaws of the wolf world(whatever its called) shot the beam line across them, which actually landed on most of the big nids,(I was running around a hill so they were lined up as strait as could be) they ALL failed their initiative test, and died. Lost 1400 points instantly.
7. playing as tyranids with a unit of 30 termagaunts, my opponent used the slaanesh ability to move a unit with both of his Daemon princes(5th edition), he managed to move them into dangerous terrain, which I rolled miserably to save them, and lost 23 of the 30 Termagaunts.

Ugh.
1: My Master of Ordnance scattered his blast across over 30" of board and wiped out five Veterans. Not self destruction you say? Do you see that model anywhere now says I?
2: A Veteran section with demolitions manages to blow itself up... Again.
3: My IG have an Infantry/Veteran section dubbed 'Red Section' with a plasmagunner. In almost every game he has been in his first shot has blown him sky high - in fact his kill count stands at less than 10 figures even though he has been in countless battles.
4: Lucius the Eternal. Turn two and he enters combat and beasts the enemy champion only to turn into a spawn and die shortly thereafter.
