What is the closest thing to a portable and/or vehicle mounted fusion reactor in the 40k universe? I have an idea for a build and this bit of tech would really make it fit that much better. If such a thing exists are there any models where it is featured?

Looks like it is, according to the wiki.
The main power source for the suit of Power Armour is located in the suit's backpack. This highly-efficient sub-atomic microfusion reactor core provides the power for all internal systems.

Handheld fusion plants are literally everywhere. I think the skitarii have them too, in addition to literally every single person in power armor.

Not necessarily. In the RPG fluff Astartes power armour is powered by a reactor in the backpack but the power armour used by Battle Sisters, Inquisitors, and other non-cyborg-super-soldier people has batteries rather than a self-contained reactor. It may be an issue of mass-producability rather than an issue of weight/size, though. As to Skitarii they don't carry reactors, the "radiation" debuff aura on the Vanguard is there because their ammunition is deliberately very toxic hot fuel.
Off the top of my head the only other models I can think of that explicitly carry onboard reactors are Scyllax, which are 30k Mechanicum bots that are a bit bigger than Marines and live on 32mm bases. I think it'd be safe to assume that something the size of a Space Marine backpack could contain a fusion reactor but maybe not much smaller than that.

I think the only source that indicates that SoB armor might not have a power supply is Dark Heresy, where Chaos Space Marine armor also doesn't have a fusion supply, so it's definitely wrong on that account and is probably a "balance" measure.
Considering that SoB have very similar backpacks to the space marines with just a couple of flourishes, I'm 99% certain that they do and it's probably just people without backpacks [like some of the Inquisitor model] that don't have the fusion packs.
