Entirely forget about your unit of Scouts which could’ve won the game if you hadn’t only sprayed them Dark Angels Green, deployed them on a wood close, but not on, an objective your opponent couldn’t get to, and then entirely forget about them for the entirety of the game.
I’d like to claim my 7,000.

Tying achievements to purchases would be pretty evil, even for GW. Some players are completionist enough to go for them.

Drop Site Massacre - Destroy an enemy unit on the same turn it entered the battlefield via Rapid Ingress.
Area Denial - On turn 3, force your opponent to bring in a Deep Strike unit from their own battlefield edge, after trying and failing to find another valid spot to deploy it.
Blur of Speed - Have a non-Aircraft unit end a turn 24'' away from where it started it, without using any teleport or redeploy abilities.
Point-Blank Slaughter - Destroy a unit within engagement range with weapons that have the Pistol rule. (Vehicles don't count, it would be too easy.)
I'll Take Those Odds - On turn 1, draw a Secondary Objective card that you are allowed to shuffle back into the deck, but keep it anyway... and achieve the objective.

War Crimes, Evidence of - 5,000G - Table your opponent. Take photos of the event and post them online for others to enjoy!
Okay, in defense of my now surprisingly high achievement score. It was to prove a point on how fast and aggressive Kroot are. A lot of people in the competitive Tau discord were still not believing how good the full Kroot Detachment was, so I posted a bunch of pics of me tabling a Grey Knights player in 2 turns. (Also, I forewarned him not to deploy aggressively on the line or put 1/4 or his army in Deep Strike and he didn't' listen, so he certainly didn't HAVE to get tabled turn two, he kinda played into it.)

It's far less entertaining than what you're looking for, but the Crusade content in every dex has three achievement levels based on whatever that faction's long term goals are.
There's Bronze, Silver and Gold, so if you Crusade with a single roster long enough, you will achieve all three.

Cultured 500 - move from collecting your intro marine army to a xenos army and discover the joys of complaining about how your current favourite army is ignored by GW and it would have been easier to stick with marines but you're much more mature and sophisticated now and marines just don't have the oomph to keep you intellectually interested so you have to rely on pure willpower to enjoy the faction you now play. Feel superior to everyone around you while you whinge incessantly.

Ooh, I'm glad I'm not the only one who ever thought of this. Here goes with possibly a long list of mine (we'll see how long I go until I get tired or can't think of more). Apologies if I do some that have already been done earlier in the thread, we'll just chalk it up to great minds think alike, I'm not going to deliberately copy anyone's homework! Some of the names might be familiar to Call of Duty players too, but that's just because they're awesome. All objectives must be done in a single game, unless otherwise stated.
General Achievements (can get them with any army, theoretically):
Flawless Victory: Win a game without losing a single unit (individual models can die/lose wounds, but no whole units were destroyed).
Peacehammer 40k: Complete (win or lose) a game without a unit on either side being destroyed.
Flyswatter: Kill an Aircraft model that is not in hover mode, or a flying monster with 12+ wounds.
Total Annihilation: Win a game after tabling your opponent.
From Beyond the Grave: Win a game after being tabled.
Old School: Destroy an enemy unit in the first battle round, kill the opponent's warlord, and have at least one unit in the enemy deployment zone at the end of the game.
HeroHammer: Destroy 4+ enemy units with non-Monster, non-Vehicle characters.
The Return of Smashf*cker: Destroy 3+ units with one non-Monster, non-Vehicle character.
Roadkill: Destroy an enemy non-vehicle unit with a non-Walker vehicle's melee attack.
Bumper Cars: Destroy an enemy vehicle with a non-Walker vehicle's melee attack.
All your base are belong to us: Hold all objectives in your opponent's deployment zone and have 3+ units in that zone.
Domination: Hold all objectives at once (must be 4+ objectives).
The Red Baron: Destroy 3+ enemy Aircraft models with a single Aircraft model.
Crab Meat: Destroy 4+ units with a single vehicle model (Knights love this one).
Wargasm: Score both of your secondary objective cards (if playing tactical) or score 6+ points on each (if playing fixed) while also scoring the maximum primary points in the same turn.
Hulk Smash: Destroy 3+ vehicle models with a single Monster model.
Space Marines Achievements:
And They Shall Know No Fear: Win a game while not failing any Battleshock tests.
Pave Low: Destroy 2+ units in a single turn with a single Stormraven Gunship.
Bolter Drill: Destroy a unit with 20+ wounds with only bolt weapons.
Transhuman Physiology: Pass 5 or more Feel No Pain rolls against a single enemy unit's attacks (shooting or melee).
BRRRRT!: Destroy 10 or more enemy models with Assault Cannons, Onslaught Gatling Cannons, or Heavy Onslaught Gatling Cannons (and/or twin linked versions of those) in a single phase.
Who's the Girly Man now?: Destroy 3+ enemy units with Roboute Guilliman doing all damage to them.
Sibling Rivalry: Destroy any Primarch or Daemon Primarch with Roboute Guilliman.
The Emperor Protects: Successfully pass all saves vs. an enemy unit's attacks (minimum 10 saves).
Dark Angels:
Speed of the Raven: Win a battle with an army consisting of only Ravenwing units.
Elitest of the Elite: Destroy 6+ enemy units with Deathwing Knights.
Got him!: Destroy Cypher with a Dark Angels character.
Redemption: Destroy any Heretic Astartes unit with Inner Circle Companions.
Weapons from the Dark Age: Destroy 3+ enemy units using Plasma weapons.
The Kitten: Have Lion El'Jonson die without inflicting any damage on enemies.
Owned: Kill an enemy character with a mortal wound inflicted by Lion's shield (no longer possible under his new rules, but I had Lion do that in a game once and it was pretty funny so I had to include it).
Tyranids:
Zerg Rush: Charge an enemy unit while it is in your opponent's deployment zone on turn 1.
Stolen Jeans: Destroy 3+ enemy units with Genestealers.

Cheater: Social Manipulator - 550G - Your opponent has zero chance of seeing you without your clothes, but wants to. You use this to your advantage and flirt mercilessly with your victim. Your opponent cannot concentrate on the game, but still goes home alone.
This reminds me of a guy playing in events at one of my FLGS. For events with good prices, he would pay a good looking girl with revealing clothes to hang around near his games to improve his chances of winning.
7 Ork facts people always get wrong: Ragnar did not win against Thrakka, but suffered two crushing defeats within a few days of each other. A lasgun is powerful enough to sever an ork's appendage or head in a single, well aimed shot. Orks meks have a better understanding of electrics and mechanics than most Tech Priests. Orks actually do not think that purple makes them harder to see. The joke was made canon by Alex Stewart's Caphias Cain books. Gharkull Blackfang did not even come close to killing the emperor. Orks can be corrupted by chaos, but few of them have any interest in what chaos offers. Orks do not have the power of believe.

Okay, so seeing more and more of these, I got a little curious to how many I'd have since I've been playing for a long time, but here and there until the last couple years, so I'd consider myself a reasonably average player in terms of number of games I get in: (I'll try to weed out duplicates)

Tarpit — 15G — force a non-Battleshocked enemy unit to take a Desperate Escape test. (Achieved)
Sentinel — 10G — Control the same objective marker for 5 Battle Rounds in a row. (Achieved)
Tabled — 40G — Destroy all units your opponent controls before Battle Round 5. (Achieved)
Dominated — 30G — Control all objective markers at the end of the round. (Achieved)
One Man Army — 25G — Have a Character unit destroy 4 or more enemy units in a single turn. (Not Achieved) I'm not sure I've ever even seen this happen, at most I've seen Angron kill 3 in one go.
Unbreakable — 20G — Have a single unit take and pass 3 consecutive Battleshock tests in a single match. (If autopass counts, I know for sure I've gotten it, if it doesn't I'm reasonably sure I have)
Consolation Prize — 0G — Lose a match on Battle Round 5 without conceding. (Achieved, I only concede if time is a factor)
Full Spectrum Warrior — 100G — Win a match with each 40k faction. (Getting there. Only factions I haven't yet are Votaan WE, EC, and Deamons)
Veteran Commander — 75G — Use each stratagem in 40k at least once. (Okay... that's a lot, certainly haven't done this.)
That's 40k Baby — 5G — Have a unit with 2+ Ballistic Skill or 2+ Weapon Skill miss all of their attacks (outside of Overwatch). (Done, and re-rolls, with my Vindicare on FAR too many occasions)
Herohammer — 20G — Build and play with a 40k army with 5 or more Epic Hero units in your roster. (Achieved, but only because all 4 Assassins are Epic heroes and I run Karamazov as well.)
Objective Player — 30G — Win a match while you had no units on the battlefield. (Achieved)
Sole Survivor — 15G — At the end of a turn, have only one model left in a multi-model unit. (Achieved)
Divine Warrior - 1000G - Build and paint all of your minis. (ROFL, nope. All built, but I have far to many to paint them all. Some armies will eventually be complete, but I doubt my entire collection ever will be, at least not in the next decade.)
Keeper of the Black Library 666G - Write a detailed and cohesive backstory for your army. (Achieved I've got lots on both my custom Space Marine chapter The Void Rangers, and my custom Kroot Kindred, The Talons of Vawk)
Rampage 20G: have one unit single-handedly destroy at least one enemy unit every battle round. (Achieved)
See another day 10G: have your warlord survive the game with 1 wound remaining. (Hmm, I don't think I have this one. Character for sure, but I don't know if I specifically have with a Warlord.)
Brought a lasgun to a knife fight 15G: destroy an enemy model with a friendly unit in engagement range using a pistol weapon. (Achieved)
Brought a knife to a lasgun fight 10G: destroy a model with a melee attack after it used the stratagem overwatch. (Achieved)
Do you have a statue of yourself? 20G: win against a ork army with trazyn the infinite as your warlord and share your achievement with a player who has orikan the diviner. (I feel compelled to do this now, I do have Trazyn)
Expensive! 10G: have an army consisting on a single model worth 2000 points or more. (Not Achieved. Most points intensive model I have is a Falchion, used to be a Stompa, but I sold my Orks a while back.)
Two old men fighting 100G: have two character models fight each other for five battle rounds, yet somehow not inflict a single wound on each other. (This is so hard XD not only to get them in combat round one, but the lack of wounds. I've had a Guardsman Character without a melee weapon 1v1 a Tau Cadre Fireblade before and even they managed to eventually hurt each other.)
Darktide 20G: destroy a beast of nurgle, daemon host, or plague ogryn with a primaris psyker, ogyrn, kasrkin, or ministorum priest. (Not Acheived... yet, but I am playing a friend of mines Deamon list with Beasts of Nurgle soon vs my Guard army with some Kasrkins, so maybe coming up.)
Gets hot! 10G: have a model die from using a hazardous weapon. (Achieved)
Big Boy - 1,000,000. Manage to complete a game without whining about your own codex. (Achieved. I know the meme, and this is pretty funny, but I've actually rarely heard people do this in the wild.)
The divine 20G: have orikan the diviner use The Stars Are Right and destroy a deciever shard unit with orikan or a unit he is attached to. (Really liking these lore related ones, but sadly, Not achieved)
Brutally Kunnin 20G: destroy a warlord titan with a shock jump dragster. (Not Achieved)
250 kroot hounds 250G: have 250 kroot hounds as a army, and win. You certainly will not regret fielding 250 kroot hounds. (Yes I know not legal army but could have a mode with 0 restrictions in private games) (It's funny, I was in an Apoc game recently, were they said there were no army restrictions and specifically called out that you could field 250 Kroot Hounds if you wanted to, If I had them I would have, but I only have 20. I did field a full 3k of Kroot though, so that's about as close as I think anyone's gunna get.)
Da revolusion 30G: kill a ork warlord with da red gobbo. (Nice, but also Not Achieved)
Is that a real army? 20G: make a imperial agents army. (Achieved, and I've won with it too.)
Design your own 6 mission campaign tree. (Achieved)
Complete a narrative campaign (Achieved, but not one I've made sadly, they tend to be really long and life gets in the way.)
Level up one unit, gaining a veteran skill, with experience points earned in a campaign (Achieved)
Spread the Wealth - 150G - Pay somebody else to paint your army, and admit it to others. (Still technically Not Achieved, though I did trade someone some of my Death Guard in exchange for them painting a bunch of my terrain.)
Meltdown - -500G - Ragequit a game. Must throw dice, models, books, templates, or food. Incoherent Screaming Optional. (Seen it happen against me, but I have Not Achieved)
Truly Evil - 100G - You must kill your opponent's newest and favorite model first, before it has a chance to do anything. (Achieved, and funnily enough, while traying not to, without looking like I was trying not to, just through a couple lascannons at it and rolled real hot.)
New Dice -10G- Roll a double 1 when making a Charge Roll, re-roll it and roll a double 1 again. (Not Achieved, but close, rolled 3 into 3 on a 4" charge once.)
Steely Aim - Destroy a charging unit with Overwatch. (Achieved)
Long Bomb - Successfully perform a 11''+ charge. (Achieved, made a 12" once)
Ford Pinto - Suffer a Deadly Demise three times in one game without using a stratagem to that effect. (ROFL, Not Achieved though, I tend not to run too vehicle heavy, so my chanced to get this are rare.)
Self-Destruction - Lose a unit to the Hazardous rule. (Model yes, unit no, Not Achieved.)
Mutual Destruction - Destroy an enemy unit with a unit that has already lost its last Wound (e.g. via Hazardous, Deadly Demise, a counter-attack upon death, etc.). (Not Achieved, but I have had it happen to my, my War Shaper took out a War Dog once, but died to the explosion.)
Death of a Thousand Cuts - Take away the last Wound of a 10+ W model with a Dmg 1 weapon. (Achieved in spades, I've downed Knights with mass D1 fire on multiple occasions.)
Seasoned Commander - Play a game against 10 different factions. (Achieved)
Conqueror of a Hundred Worlds - Win a game against every playable faction. (Before Emperor's Children came out, I would have had this, but I haven't played against them yet.)
What If I Need This Later? - Finish a game with 5 unspent Command Points (not including games in which the opponent conceded before the end). (Not Achieved)
Clumsy - Accidentally knock over or aside a piece of terrain that has models under or on it. (Achieved, RIP my plastic Callidus, we never found where her pistol flew off to.)
Frankenstein's Model - Build a model using bits from at least three different factions. (Not Achieved, I think 2 is still my max.)
Sportsmanship - Remind an experienced opponent of a rule to their advantage that they were about to forget. Alternatively, allow an opponent to perform a move, an attack or a charge after the appropriate phase has ended. (Achieved, on a regular basis.)
Never tell me the odds - roll ones on 5 rolls in a row (or the entire batch if fast rolling 5+ dice) (Achieved)
Seizing defeat from the jaws of victory - loose a game where you were ahead on points for the first 4 turns. (Achieved, though not this edition.)
Poor Sportsman - 50G - Remind your opponent of how awesome you are playing in the game, I bet they are unaware. (If a no stakes pop& chips game with my best friend counts, then Achieved, otherwise Not Achieved)

Unhelpful Suggestion - 5G - Point out to your opponent a totally obvious maneuver that they were going to make, and then act poorly when they don't thank you for your tactical genius. (Probably the first part at some point, I help coach a lot of people, so it's hard to turn it off. Not the second part though.)
"It's a Titan Karl!, Fix BAYONETS!" - ???G - Destroy a Warlord Titan with IG Infantry in melee. (Closest I've come is a Riptide, but now I really want to try.)
Ork achievements: (Don't have an Ork army anymore, but I like the faction specific ones, might write some up of my own)
Let the Wookie Win - 75G - When you are playing your significant other (or someone who hope to see wearing less clothing), you throw the game. Wookies rip people's arms off when they lose, or so I hear. (I didn't throw the game, but I did take it easy on my SO for her first game and she won by 1 point. So... kinda?)
Cheater: Speed Dice - 50G - You rolled and scooped. Nope, there were definitely no 1's in that roll. (Not intentionally, but I know when playing a few times I've been distracted and scooped up dice without thinking/looking at them, so... possibly Achieved)
Cheater: Extra Wounds - 150G - Yep, your Tyranid Warrior just got 8 devastating wounds, but he's not dead. He's got 1 wound remaining. Sure that amount of damage killed your other ones, but this guy's important, he's sitting on an objective. (I've done this for an opponent in a Narrative game I was running away with without them realizing, but not for my own units, so... Kinda.)
Cheater: Poor Math Skills - 250G - Let's see, you've taken 16 wounds to you Astra Militarum Infantry Unit, made 7 saves... carry the 2... yep, 5 died... no need for a Battleshock test... You're math skills will win you the game! ( Not Achieved)
Cheater: Points Handicap - 500G - It's a friendly game, no need to look too closely at my handwritten army list, dear friend... no reason to ponder how I've fit in another 450 points of Terminators... no need to question it... (I've played some points imbalanced Narrative games, but that was never a hidden fact, so... another kinda.)

Cheater: Rules Interpreter 200G - Did you miss the latest update? No, it's not on the App Yet... yeah, you can't charge me on the first turn. I understand how it's easy to miss these things... I'll let you do something else. (Not exactly this, but I was incorrectly using one of my Legends models for half an edition. None of my opponents noticed, it was actually someone on Dakka that clued me in to my mistake when I was talking about one of my games.)
Table Diva - 5,000G - This is my table! The store always reserves this table for me! No. I don't want that one over there! This table is closer to the wall outlet, so I can charge my phone. No, I will not play on that table over there! (ROFL, Not Achieved.)

TLDR. They're all pretty good. Generalist ones that are easy o get, weirdly specific ones, joke ones. I really like the really specific narrative ones though. I think that would get the most people hyped if this were ever somehow a thing.

Horus Heresy Redux - Have a fight to the death between a loyalist Primarch and a traitor Primarch or Abaddon. (Both players get this achievement regardless of who wins the fight.) (Am literally in the middle of an Apoc game (paused for the week) where this is currently happening. Omegon (using Guilliman's rules for him) has just ambushed Mortarion.)
Swearing is for Sore Losers - Lose a game with a >30 VP difference without once uttering a word that would get a movie's MPAA rating bumped from G to PG-13. (Hmm... I've only lost a couple games in recent memory by that much and I don't recall if I swore of not in them. 50/50.)
Antiquarian - Field an army that does not include any kit more recent than 4th Edition. (Achieved. Ran a 2k Firstborn army with old Boxnaughts and everything.)
Recruiter - Introduce a friend or significant other to the hobby. (Achieved. Took me nearly 15 years to get my best friend into the hobby, but now he loves it has has 4 armies. My wife has played all of once, so I'm not sure that counts.)
Legacy Keeper - Introduce your child to the hobby. (Achieved x3. All my kids play. My oldest son pays Orks, my second son White Scars, and my Daughter Tyranids.)
Oldhammer - 20G - Deploy an unit entirely composed of miniatures released pre-2000. (I'm pretty sure the old 25mm Terminators and the metal scouts were pre-2000. If so, Achieved.)
Cultured 500 - move from collecting your intro marine army to a xenos army and discover the joys of complaining about how your current favourite army is ignored by GW and it would have been easier to stick with marines but you're much more mature and sophisticated now and marines just don't have the oomph to keep you intellectually interested so you have to rely on pure willpower to enjoy the faction you now play. Feel superior to everyone around you while you whinge incessantly. (Achieved, (though not exactly in those "I'm too cool for marines" words, I still play my marines) in previous editions. I'm really happy with all the love Kroot got in 10th, so no complains from me this time.)
Flawless Victory: Win a game without losing a single unit (individual models can die/lose wounds, but no whole units were destroyed). (Achieved, once with full Kroot too if you can believe that.)
Peacehammer 40k: Complete (win or lose) a game without a unit on either side being destroyed. (Not Achieved. Can't imagine this could be done without both players deciding to do so, though perhaps that is the point.)
Flyswatter: Kill an Aircraft model that is not in hover mode, or a flying monster with 12+ wounds. (Achieved.)
Old School: Destroy an enemy unit in the first battle round, kill the opponent's warlord, and have at least one unit in the enemy deployment zone at the end of the game. (Achieved.)
HeroHammer: Destroy 4+ enemy units with non-Monster, non-Vehicle characters. (Achieved.)
The Return of Smashf*cker: Destroy 3+ units with one non-Monster, non-Vehicle character. (Not Acheived. I tend to to run crazy large super fighty characters. I've had chars finish off a unit and a character in one go, but not a 3rd.)
Roadkill: Destroy an enemy non-vehicle unit with a non-Walker vehicle's melee attack. (I've had this happen to me, but I don't run vehicle heavy too often, so I can't recall a game where I would have got this one.)
Bumper Cars: Destroy an enemy vehicle with a non-Walker vehicle's melee attack. (if Tank Shock counted, I'd have this. Otherwise Not Achieved.)
All your base are belong to us: Hold all objectives in your opponent's deployment zone and have 3+ units in that zone. (Achieved)
Domination: Hold all objectives at once (must be 4+ objectives). (Achieved)
The Red Baron: Destroy 3+ enemy Aircraft models with a single Aircraft model. (Not Achieved. I own all of 3 aircraft and I almost never use them.)
Crab Meat: Destroy 4+ units with a single vehicle model (Knights love this one). (Achieved, my Falchion has a pretty impressive kill count.)
Wargasm: Score both of your secondary objective cards (if playing tactical) or score 6+ points on each (if playing fixed) while also scoring the maximum primary points in the same turn. (Achieved.)
Hulk Smash: Destroy 3+ vehicle models with a single Monster model. (I don't think I've gotten this one either. I don't run monsters too much, and I don't go up against vehicle heavy lists too much either, at least outside of competitive where I play Kroot.)
Space Marines Achievements:
And They Shall Know No Fear: Win a game while not failing any Battleshock tests. (Achieved.)
Pave Low: Destroy 2+ units in a single turn with a single Stormraven Gunship. (Hey it's one of the two flying units I have. Achieved in an Apoc game where it split fired into some small injured Tyranid units.)
Bolter Drill: Destroy a unit with 20+ wounds with only bolt weapons. (Not Achvied. I've certainly inflicted 20 wounds with bolt weapons to big targets in a game, but haven't exclusively downed a unit with them.)
Transhuman Physiology: Pass 5 or more Feel No Pain rolls against a single enemy unit's attacks (shooting or melee). (Hmm... don't think I have this one from what I can recall.)
BRRRRT!: Destroy 10 or more enemy models with Assault Cannons, Onslaught Gatling Cannons, or Heavy Onslaught Gatling Cannons (and/or twin linked versions of those) in a single phase. (Hard to be sure, I don't think so, but I've probably come close.)
Who's the Girly Man now?: Destroy 3+ enemy units with Roboute Guilliman doing all damage to them. (Not Achieved.)
Sibling Rivalry: Destroy any Primarch or Daemon Primarch with Roboute Guilliman. (In the middle of an Apoc game where this may be about to happen, otherwise Not Achieved.)
The Emperor Protects: Successfully pass all saves vs. an enemy unit's attacks (minimum 10 saves). (Achieved. 21 2+'s on Terminators is my record.) (I've seen someone make 23 3+ s on Marines once before, it was glorious.)
Dark Angels: (I don't play Dark Angels.)
Tyranids:
Zerg Rush: Charge an enemy unit while it is in your opponent's deployment zone on turn 1. (Achived.)
Stolen Jeans: Destroy 3+ enemy units with Genestealers. (Achieved, in the same game as above.)
Man, going over all these really have been taking me down memory lane. I think this might be my favourite thread. Lets keep it going.

Unlocked! My (now-retired) Daemon Prince was converted from an old metal Prince, using a head from a Great Eagle, wings from a Pegasus Knight, a tail from a War Altar, and hands from the plastic dragons (and a staff from a Lord of Change, but that one doesn't count).
Not sure if I’ve scored this one.
Converted up a Savage Orc Warboss on Wyvern using the metal Wyvern kit, Orc Boss kit, and High Elf Dragon kit. The third ‘faction’ was General Modelling String, used to show the Warboss had been tied to the Wyvern (like a hang glider).
