It says something like "may replace bolter with:". Does this mean i can only take that weapon and a bolt pistol?

If an army list entry only lets you replace your bolter, then you can only take one such option (as you only have one bolter to replace).

If that is how it's worded, yes.

If it says "may replace bolter and/or bolt pistol", then you can do both, either, or neither.

So does everybody play wrong? I have seen so many sergeants with plasma+power fist

I don't have my book on hand to look but:

Short answer: if both plasma pistol and power fist require you to replace your bolter then yes, it's illegal.

However it seems to me (again, no book to check) that the plasma pistol would replace the bolt pistol, in which case the loadout is legal.

Which version of the rules are you looking at, and which units?

The ones i've looked at seem to give you either the option to exchange your boltpistol and/or chainswod, or have separate lines to exchange bolter or chainsword.

Book 1: betrayal. it says "The legion tactical squad's sergeant may exchange their bolter for one of the following"

Book 1's Army list is outdated.

I'm looking at the current army list book now. The tactical squad sergeant entry states

"may exchange their bolter and/or chainsword for one of the following:"

Other sergeant entries break it down with "replace bolter with X", "replace bolt pistol with Y", or similar organization. Offhand I don't see a sergeant entry that would cause this issue...?

Edit: Ninja'ed. The book 1 list is outdated by two editions.

This book, also has updated points costs and rules.

The Sergeant entry reads "the Sergeant may replace their Bolter and/or close combat weapon with one of the following"

Because of that you can replace one or both weapons with one in the list.

You can take a Plasma Pistol AND a power fist
OR
You can take the Plasma Pistol OR the powerfist and keep either your bolter OR close combat weapon.

It's so that you flexibility in your choices.

Remember that it also depends on the unit! Sergeants in different units have different wargear options.
