To get the ball rolling I played a game recently where my 2 x PF hellbrute went one on one with my opponents Libby (warlord) in the last turn of the game. I practically only had to hit the guy once to get slay the warlord and an another kill point and win the game. I rolled 3 1's followed by him melta bombing me. Turning a draw into 1 vp loss.

A game last weekend was not going well. The dice gods were favoring my opponent. With my sternguard dead, my librarian is making a run for it, to try to keep ahead of an angry Ork mob and prevent them from getting the Slay the Warlord VP.
I've got GoI, so I just teleport away, right? Psychic test comes up 11. Well, at least I didn't perils. We have a laugh over that, it's been that kind of game. So I stagger though some difficult ground, forgo shooting to try to run farther, and end up about 4" from where I started. Conveniently just outside the cover I was hiding in. Needless to say, the following turn was the last one for the poor guy.

I'm rather famous at my LGS for my bad rolls. I almost never hit with Meltaguns, so I'm starting to just run flamers. Though I have failed to wound with them pretty often.
The worst though was terminators getting hit with 6 Str 4 AP 6 wounds. Should be a easy save, I might lose one situation. So I scoop up the dice and utter those fatesealing words, " Just need anything but ones." I precede to roll 6 ones. Epic bad.
Though in another game I needed to make 3 5+ saves and managed to roll all 6's. The joke being I make the almost impossible saves, but with fail on the ones that should be so easy to make.

Go all-out and see how far you can get on the skin of your teeth.
I once had a squad of 7 Eldar Pathfinders lined up to shoot a low-points enemy unit.
They line up the shot, take it, and ALL roll 1s.
What's the odds of 7 1s?
I didn't win that game, but being Eldar, I'm used to it.

I was playing my friends Crimson Fists with my DA a couple of days ago. Big 2500pt game. For 3 turns, a DW Sergeant with PW/SB was in CC with a normal marine sergeant. I needed one hit to kill, and did nothing for 3 turns. He finally kills the marine on turn 5 for a tabling.
That same DW Sergeant has, in a previous game, survived 2 turns of shooting from the allied remnants of a Crimson Fists and Necrons force. They were shooting at just him, his squad was dead. Around 700pts of models shooting at him.
Seems he can take the hits, but not dish them out!

My platoon commander (a lieutenant) brought down my friend's warlord (a Chaos Lord) 2 chosen and then contributed to bringing down a hellbrute with a volley of Krak grenades. He then dies and I lose the game to 3 cultists and one remaining chosen.

Back in 5th:
Kharn and 8 berserkers charge Vulkan who is by himself after his termi escort died to lots of plasma.
Kharn kills 5 (!) pals at I6, while Vulkan makes all his saves. Vulkan then pulps the rest of the squad (I5), leaving Kharn alone. Losing combat by a LOT, he has to make a ton of saves to avoid Fearless wounds, and fails 3 of them.
I still managed to pull out a win thanks to some desperate contesting with barely-alive vindicators and a Daemon Prince, but it was a colossal bungle.

Wasn't my fail, but the fail of a guy playing against me using DE, I was using SM
At the start of the game I said that my CC terminators were packed into a LRC with a chaplain.
Bottom of turn two he moves his talos across the board to try to kill one of my units... and he puts it about 4 inches from the LRC... So 7 LC terminators and the chaplain get out, walk over to the talos and proceed to make it a crater thanks to rerolled hits and misses....
Now when it comes to me failing?
The dice gods like to make my deep strikes into dangerous terrain.....a lot..... And ive had a leeman russ hit itself with a bad scatter

I decided it would be a good idea to have a unit of 10 lootas in a ruin next to the ruin holding my Shokk Attack Gun and accompanying grot squad.
It happened, I rolled the double 1 with a radius of 6" and lost all my lootas and my big-mek.... on turn 1. Nothing starts a 1500 point game out right like a ~275ish point handicap.

In 5th my regular Tau opponent used to fail constantly. He would regularly have a hammerhead and multiple broadsides, but could never roll higher than a 1 or 2 on the vehicle damage charts. It would have some effect if he wasn't playing me and trying to kill my daemon engines all possessed and automatically ignoring all shaken and stunned results. Multiple games in a row my plague hulk or defiler would walk all the way across the table being hit 2-3 times a turn with railguns and never suffering any ill effects from it. They would then proceed to assault the broadsides and destroy them and then assault the hammerhead and destroy it. Was funny the first and 2nd time and just depressing for him every other time after that. In one game he even ran in behind my defiler with one of his light skimmers with twin linked meltas on it. Rolled a 1 to hit, and then on the reroll rolled another 1 so couldn't even kill it that way. The funny thing is that his fire warriors do far better than his big guns do. He markerlights the heck out of a unit and shoots it to pieces, then moves to the next unit and repeats.
I haven't had a chance to play him since 6th came out, but he will be much happier now that even if he only rolls a 1 or 2 on the vehicle damage chart he will still take away a kill point, so 4 hits and the plague hulk is gone now instead of effectively surviving forever due to poor rolling. Of course I am expecting now that it doesn't matter he will roll better on the vehicle damage chart.

My third game of 40k ever in early 6th Edition, and I think I have the situation well in hand by having surrounded the enemy Chaos Lord with my 3 TH+ SS termies and 2 LC termies, while his lord only had a single aspiring champion or some such with him after my shooty terminator squad had killed the rest of them, while getting killed in turn by the Lord. But as you might expect, the Lord, with his AP3 Daemon weapon (if I recall correctly) of course proceeded to kill the entire terminator squad, and only the Sergeant died. The Lord did take two wounds, but that was hardly much consolation.

Playing a game of the scouring with a buddy. We use all of my models for both armies because he doesn't have any.
He hits his first 13 of 15 shots fired on his first turn, including lascannon and autocannon shots from a predator. He his 100% of his wounds. I proceed to fail 60% of my 3+ armor saves.
The next turn I use sternguard veterans to blow up a predator with melta bombs. Hits, explodes, roll wounds on all three sternguards in range, fail all three armor saves.
I was furious.

First turn, sending the Drop Pods in, had one Pod with a ten-man Sternguard squad, with ten combi-weps, plus a Power Fist, plus Lysander, all scatter off the table and die. (Over five hundred points lost in a single roll.) I almost wept.

Played an armoured Compay against a screamer/flamer list before I knew what the update had done.
Good news was it was over by turn 2, so I could go play a real game.
My record for failure was vs GK in 5th. I had a normal guard list, plenty of AP2, some artillery, some tanks, fair number of infantry and a vendetta, and my opponent was running a henchmen list, mostly in razorbacks and chimeras, with a single unit of GK Terminators, hardly a cheese list.
I managed to wreck one razorback, and got tabled. My opponent actually facepalmed, while exclaiming "that's not supposed to happen."

I was running LRBTs in a squadron back in 5th. I had to back the squadron up to avoid getting assaulted and the path of one of the russes led into difficult terrain. So I rolled my dangerous terrain test,  the normally means immobilized, but in a squadron immobilized is destroyed. So I lost a russs to a tree.

It was 7500 points a side and I got to bring out my shiny Dominatrix. My friend took a vortex grenade as a strategic asset, because he just thought they were hilarious.
He runs out with some lone guy in the face of the full health-wound regenerating Dominatrix and chucks the nade. It then proceeds to chase the Dominatrix for the next two movement phases, scattering 11" to keep up with her.
Needless to say, she got a single round of shooting off before my 1750 point centerpiece was sucked into a mini black hole. Best part is the vortex grenade vanished after that.
Talk about efficiency haha.

An Ig player I know loves to take leman russes in his list. Went to a local tourney where once where there were custom scenarios back in 5th ed. He has 6 leman russes in 3 squadrons.
The special scenario was the entire board is difficult terrain.
Everything had to walk on the board from your table edge turn one.
He lost half his tanks just by moving them onto the board

I think you've told this story before and it makes me sad every time I see it.
My most epic fail was a 2k game. I forgot to run my 7 termies and Libby when they deep struck. 27 wounds from a thunderfire later I lost 6 with my Libby and the last guy running off the table.

5th Ed stories
1) Ironclad with gubbinz drops in behind a Dev squad hiding behind some cover. Dev squad's 2 Lascannon buddies turn around and Freem him through the faceplate. Not the wisest move... but I can top it with:
2) Playing against a mechy Eldar list with my mismatch Marines and it wasn't going well. Turn 4 comes and I grab my reserves. Drop my Ironclad and 5 termies next to his only Fire Prism. The Fire Prism then proceeds to move 24" towards a ruin while the rest of his army blow up said Dread, the Pod and 4 Termies. I R Smart... not. It only got funny when the last Termi with a HF/ CF decided to run after a squad of Dire Avengers and a Falcon. He withstood all the firepower thrown his way and reached the Falcon for a back-door slice-up. And of course I roll 2's at best on the damage chart every turn I got a hit off. This was including his funny stones that meant I had to re-roll and take the worst result which was always a 2, which he negated to a 1. Frothing madness...
And lastly
3) Stuck in with the Marines vs pre-6th DA. Captain gets into combat with remnants of some elite unit (Veterans I think). Rolls for hit and wounding and does nothing substantial. The DA fella rolls and wounds 3 times, all negating armour. So, 3++ and 3 wounds. Aaaaand two 1's and a 2 was my result. It's like that Captain's shield was made of cardboard and tinfoil

I had a TH biker marshal and buddies, charge into a 5 man scout squad, kill one with HoW and the bikers kill another, my marshal had 4 attacks, hit with 3 and got 3 ones on the wound roll, at least i know now he'll never do that again

Played DE versus GK in a relic game.
There were 2 bastions on the field (not paid for terrain - so no guns) next to the relic plus lots of trees, hills, etc. Neat setup.
GK goes first. Teleports a squad next to the relic. My turn I push 3 raiders full of warriors near him, unleash with everything, nothing wounds. That would be over 60 poison shots and 3 DL against a 5 man strike squad.
He then moves the relic into a bastion and kills all my raiders.
At this point I have nothing that could possibly penetrate the bastions armor except for a razorwing that is still in reserve.
We take pot shots at each other until the bottom of turn 4 when my razorwing comes in. It shoots at the bastion, utterly failing to do anything. Top of turn 5 his storm raven turns and blows it out of the sky. We call the game at that point.

My wife and I played our first game together. I was plain Codex Marine, she was sisters. 500pt game with 5th edition.
I had my librarian go up the left with 5 tacs behind terrain for cover and a rhino with a larger squad go up the center. My wife flew Saint Celestine over the terrain and did her flamer ability on the librarian and his squad, 3 dead. She charged and killed the rest along with the librarian. Next turn I dropped most of her 1st Sisters unit only to be charged from the back and cleared by Saint Celestine on her next turn.
That was the worst game ever

My worst game was about two weeks ago. I was trying out some stuff in the CSM codex I haven't tried yet. I'm still not that familiar with them as a whole and just prefer them as allies really. But I decided to go all chaos while my friend went with DA bike heavy list with the dakka banner.
I don't remember the details, but he brought GK allies just to put a vindicaire assassin in a bastion with the Icarus Lascannon. One shot my heldrake. Then the same turn my Warlord, a MoT sorceror in termie armor with 4 terminators deepstruck and rolled way too high. Scattered off the table and rolled a 1 for mishap. So turn 2, haven't even ended my movement phase really and I lost 400 points of units before they could do anything.
All my other shooting was terrible, while the twinlinked salvo bolters just tore my army apart. By the end of his turn 3 I had lost 80% of my army and I only took down 1 attack bike, so I decided to call it there.
I have to ask though, because he told me I should "tough it out" Should I have kept going? I really don't see any point in playing "watch you win more" when I had literally nothing left to use. I know some people would say that I should never quit because I can always learn something. But I'm not an idiot. I saw my mistakes fairly early and realized that my list building needed a lot of work. I could always play another game and start from there. I don't see how learning to play with 20% of a bad list against a biker swarm will really teach me anything useful.
aside from this, my worst fail in 5th involved the same guy. I was playing against another player, I had Tau and the other player used SW. It was a campaign game and I really needed a good win for the campaign bonuses for the next game. I had 2/3 objectives, each worth additions to my campaign list, and well on my way to getting the third. I had outflanked a unit of kroot behind some SW with Firewarriors on the other end. I was going to shoot them with both units, he could only take out one on his turn, and the other unit would be free to finish them off the next turn.
Then in came my friend. I was fairly new and he managed to convince me how awesome Kroot are in CC and how I should charge. Well I shoot down a few with firewarriors, and declare my charge. As I'm moving in the Kroot, I look up and say "wait. Aren't SW that one army that even other marines don't charge?" He kills off a majority of my kroot before I can even hit back, I do almost no damage back, and then get swept up leaving him free to destroy the FW on his turn. Game ended with my HQ crisis suit and a lone rune priest shooting S7 shots back and forth. With no objectives I went into the next game of the campaign with only a basic army against someone with a much larger force and pretty much had to quit because the only way to unlock FOC slots was to win games so I only had 1 HQ and 2 troops to use.
I did learn one huge lesson though. There are a lot of people who give advice on how to play Tau. Unless that person actually plays Tau, disregard it. 3 attacks on the charge, S5 standard weapons, and tough transports sound awesome to people who don't have them. But when you play Tau you have no problem laughing those off.

Last game i played against my brother it was my tau vs his demons and csm allies the game was capture objectives based i killed his fate-weaver but he ended up using icons to bring in more demons then i could handle and i went from owning more then half the objectives to tabled after fate weaver went down in turn 2 after that my guys just could seem to ever hit any thing! even the marker lights never hit! btw any one know a way to un jinx dice i think the death of faitweaver cursed them .

Hah, funny story I have.
I'm running competitive Daemons against Silver Tide 'Crons.
Turn three I decide to charge into their 24" kill range and try my hand at surviving long enough to strike back. Fateweaver is leading the charge and making the other daemons survivable.
He had the stormlord and a chonotron to re-roll one dice per phase (aka nightfighting).
Fatey's flying to keep him alive. Come the shooting phase, the lightning is shooting down from the sky and he rolls for The Big Ultra Supreme Flying Chicken and gets a 1. He's like, "I don't usually re-roll a lot of things, but i'm going to give it a go." With the re-roll he nets a  and what happens? I fail the 3+ to keep him in the air.
So there fateweaver is: on the ground in front of MASSIVE buttloads of S5 and S7 shooting. all because of a friggin re-roll of a lightning strike. He died two or three volleys later. I laughed it off, and ended up tying the game.
It's always moments like this when i remember why i love this game. Anything can happen.

Just finished a local tournement where during the 2nd match I was called to the TO table to discuss my list. He built my sister/GK 1750 list in his army builder and came to 2300 points and was about to disqualify me for having too many points. Taking the news I grabbed my codex and built my list point for point with him. Not liking that I challenged him he built my list again through army builder and came up with 1900. Explaining again that can't be he decided that I was playing an illegal list using justicars without paying for points. So told me I needed to drop the personal teleported on my dreadknight and not use justicars... Went back to my table and explained to my opponent that apparently I can't make a list and showed him my list. He scoffed and didn't see a problem and went to talk to the TO and explained justicars are stock with the group. That you don't pay extra unless you upgrade to justicars thrawn. The TO then put me negative points for poor sportsmanship. We were both thrown by the decision. This took up an hour and ten minutes of our 2 hour match. We only finished turn to 3. 10 minutes before finishing he came back and said he was wrong and my points were correct. Damage was done. I was disheartened and lost 13 vp to 1 since we didn't get past turn 3. Still lost the sportsman points. I was as nice as I could through being called a cheat and a liar. Placed dead last overall over it. Really don't like army builder right now. I ran terminater group of 5 for 210pt , dreadknight for 260 and a 10 man GK strike squad for 290. Sorry for the wall of text but I consider the day as a fail.

My Daemon Prince loves to smack himself in the face with his Daemon Weapon at the most inopportune moment. He typically goes from full Wounds to Dead in one turn.
Last game three Terminators killed him because he rolled a 1 on his Daemon Weapon. Seriously considering taking the Murder Sword instead.

Since posting every single one would be insane, I'll post two great ones - one of mine, one of a fellow.
Mine: Game is Chaos Marines vs. my Space Wolves/Blood Angels. First turn of the game, I cast some blessings on my Death Company to make them a bit nastier. Prescience gets cast (I know, dumb to do it on turn 1, but hey, you never know...) aaaaaaand double 6s. Well, that's fine, it's the one power I didn't really need anyway. Let me just throw down Forewarning, give them a bit more protection, aaaaaaaaand double 6s again. Yep, first turn of the game, my Librarian blows his own brains out. Which I'm pretty sure technically gave my opponent First Blood despite the fact that I had the first turn.
Fellow Gamer's: I swear, dice tend to hate this guy, but he plows through anyway. His game was Orks (him) vs. Tyranids. Now, all I saw of this was a big, nasty assault going on, I believe with a Tervigon (those are the gaunt-spawners, right?), a Hive Tyrant, and a Tyrannofex vs. a mess of Nob Bikers, including Wazdakka and a Biker Boss. Wazdakka rolls his attacks - and wow, every one hit! All he needs are 2s to wound now, because he's got a nasty S10 klaw. And cue the 5 1s. Not a single wound.
From what I recall, that assault ended in a massive failure on both sides, because despite a ton of high strength, AP2 attacks getting tossed out, both sides passed every save they were called to make. 3 monsters and a mess of bikes, no wounds.

A few weeks ago, playing my famous CSM Death Guard army.
I am famous for my good rolls at my FLGS. HOWEVER there was this one necron Player, he played the typical all-Croissant army ( Here in Basel we call Croissants "gipfeli" so he's the Gipfeli-man).
No one likes that guy, so my friends poke me up to nuke him.
I was afraid of the gipfelis as I had only one flier, the Dragon.
However I was able to get all the gipfelis down and shred most of his Units, I had even three CSM squad leaders ascend to daemonhood! But then......the doomark (?) took a shot and killed 2 DP's and my army leader+terminator Bodyguards. I think that there was something fishy about that doomark, but still I have won. Nevertheless I am afraid of those arks now....

Playing a 1500pt game back in 5th. My opponent was playing orks and i was CSM. He was running this massive group of Nob Bikers in the middle of the table. Turn one, my defiler shoots at them, hits them all, wounds them all, they all fail their armour/invunerable/cover save. Every single ork was dead by one pie plate. Nearly half his army gone in turn 1, Khorne was pleased that day.

Playing a 3 way 15 player game (crusade of fire)
Typhus deep strikes onto a landing pad upon which are 3 broadsides, unit of dark reapers, eldar snipers and eldrad. Uses plague nova primais power thingy and nukes a couple of reapers and snipers. Eldrad tries to mind war him and doom him. Manages to abhor the witch TWICE. Then the railguns fire, wounded once passes FNP.

Right at the begining of my 40k life I faced an ork player with (no suprise) Ghazghull. he shook of a lot of my leman russ fire, but he only had one wound left. a veteran sergeant proceeded to run up to him and fired his laspistol...
Thats why I never play Ghazghull in my ork army.
a word of advice to all new players: never get cocky. this has happened on two occasions: when my friend (who wawn't new to the game) said he would own me with his tervigon, but rolled double 1's, and when a kid with 3000 points faced me in apocolypse with a smirk on his face and was beaten by 1250 points of freebooters. Yes, i'm a pirate.
If anyone has some good terminator fails then let me know so that i can convince my friend not to use them in every game he plays. ta!

Newer player has a whirlwind that can either shoot at an ironclad with rear armor (that's really close) or the squad of tau firewarriors far away. We all tell him to shoot at the firewarriors, but he ignores us and shoots at the ironclad. It scatters and hits the whirlwind, not killing it, but we had a great laugh

I always try to be cool, but I did get a little cocky in a game yesterday. The mission was ambush, and I was the attacker. MY Warlord trait gave me Furious Charge in my opponent's deployment zone, which was a 12" strip up the middle of the table. I got a little overexcited about S8 Dlord and S7 wraiths, only to lose 5 of 6 of my wraiths to silly things over the course of the game: vehicle explosions, failed saves in cc with basic marines, etc.

So I ALWAYS lose whenever I'm teaching someone to play their first game, and no I don't try to lose.
Here are some examples:
Taught one friend to play Eldar, I was playing my Orks and we were playing kill points.
-Lots and lots of mutual destruction later, I'm winning by a single kill point and it's just my Shokk Attack Gun (about 3 feet away) vs his Avatar.
-I go "why the heck not" and shoot it, then proceed to roll Zoink! and my last guy gets roflstomped by the Avatar in close combat.
Taught another friend to play oldcrons, I was playing Tau and there were two objectives.
-I can't for the life of me kill off the squad of warriors on her objective, so I am content with contesting it.
-Meanwhile, I leave it to my tank to kill off the other squad of her warriors (it was an ionhead, perfect for the job.) Killing the squad down to a single warrior and her lord.
-She proceeds to get a lucky glance and immobilize my tank that I thought was within contesting distance of the objective. On her turn she moves and just barely gets within 3" of the objective, and when I pull out the tape measure to make sure I was contesting I ended up being 1/16" of an inch too far away
I've lost to the three other people I've taught to play, but those aren't as memorable haha.

My lone Belial engaged in combat with a Lone Marine with two wounds on his character and he got snapshotted and then killed in the first round of combat.
Worst game was when I played a guy who had three land raiders full of Khorne Beserkers and Kharn, with those damned flyers that look like dragons, against my whole foot force of Dark Angels. I scored three hits on the Land Raiders for my whole force gone off the board by fourth turn -.-

Moved my land speeder typhoon across the board to take some shots at a whirlwind (which I killed) but wasn't paying attention to one of my other opponants broadside. For whatever reason he moved closer to it and blew it up, rolled for the explosion....killed the broadside as well xD

In 5ed, I played Iron Warriors. My chaos dread (plasma cannon/missil launcher) became crazed and shot my 10 chosens with mark of nurgle and 5 plasma gun killing half of the squad and 4 plasma guns.
in 6ed. Typhus and his terminator deep striked and the scatter dice placed him in perfect range of my vindicator. On shot and typhus and friends blow up in smoke.

My opponent just deployed along the middle line, leaving me only a small triangle in my corner to deploy in. My opponent was GK. He then proceeded to move up a few inches and unload every psycannon plus Angry Chair's Orbital strike on my bunched up units, killing almost 800 points in one round of shooting. Reserving would not have helped as he would have had all his psycannons in range of whatever i would bring on and Coteaz in a purifier squad.

I remember a game back in 4th with my nids and my hive tyrant was smashing through a guard infantry line and I got held and up and eventually killed by a damnable sergeant with a powerweapon. Somehow I kept missing and he kept wounding. Lead me to lost the game.

What was so bad about your list that made it horrible? On your other story, always stick to your own plan for how to use *your* army. I don't care how awesome or perfect someone else thinks a unit is for a specific job their advice does not impact my plans at all. One thing to offer advice to a total noob who *asks* for it, but keep your comments to yourself when you come over to watch my game. Your plan and the logic behind it was good. Next time most definitely trust yourself!

Sorry for the wall of text but I consider the day as a fail.

My fav cocky gamer dice failure is from D&D a LONG time ago...A creature related to a water elemental burst out of a fountain to attack the party and the GM laughed maniacally that "I only need 4s to hit you!" He then proceeded to roll nothing higher than a 3 for the entire combat ending in nasty creature stomped completely and party completely untouched.
I always forget at some critical point in a game too and say something that then turns my dice against me. Always a "DOH!" moment...

It was my first time playing my new GK army. I was playing against a good friend at the time and I just could not kill his Rhino. So finally I chose to charge his rhino with my termies. I cast hammerhand and I rolled a 12. As to my understanding at the time I thought all of them died.
The other time I was playing my necrons vs Tau and when my night scythe came on the board I dropped off my units. When it was the Tau players turn he shot at the night scythe and it crashed and killed all but the lord and 1 warrior.
Both times had me on the ground laughing.

Had a vet squad come on from reserves, shoot their plasma, then blow up and flee. I've had that happen a few times actually.
In my last game, 10 out of 14 casualties my plasma suffered were from Gets Hot. Plasma just hates me.

And all this could have been avoided if ol' Jones had of thrown the charge and not the pin...

Here's a Terminator fail for you: Tournament under 4th edition rules Dark Angels v. Orks (third ed. Codex). Some of you may see where this is going. Dude was running Deathwing, he deep-strikes right near 30 boys with slugga/choppa. Back then choppas reduced all armor saves to 4+. Ork player charges Termies, a hail of 90 choppa attacks descends upon the Deathwing. No more Deathwing.

Inside each transport was a mob of burnaz, they all disembarked and fired on me.

First, and only time to date I have fielding Abaddon, I rolled a 1 for his Demon Weapon as I moved into assault. This was in an apocalypse game in 5th edition, and I have since blamed the removal of his topknot for the failed attempt.

I was playing chaos.
He deployed his forces in the middle of the field, right on where the objective had been placed.

My most memerable fail was I was trying out a foot slogging DE list using my Dark elf models as I didnt want to commit to the army unless I liked it. I played against a good friend of mine who was running mech orks. My Dark lances failed to do ANYTHING at all then in combat my Archon and Incubi fail to kill any orks in return they get faced stomped

Buddy of mine was running FNP Hammernators right when the BA dex first came out. He's always been big into Elite armies...so anyway, double Land Raiders and a Storm Raven, that kind of thing.
Playing against my Guard, 2 objectives on the table...we'd always had close games but we'd tied the majority of them. He'd never beaten me. This time, he rams a Land Raider straight for the objective and melta guns are bouncing off this thing like no tomorrow. The Hammernators hop out and beat the crap out of a couple of my vehicles. Nuts...
I've got 8 enemy Hammernators with FNP sitting on an objective. They shrug off all of the fire that I can put onto them. Finally the only thing left is the Hydras. 3 Hydras in the backfield light them up. 8 wounds. 8 failed saves. 8 failed FNPs. Priest is the only one left alive. We look at each other like wtf did that REALLY just happen?

Some great stories in this thread.
This was not my fail, it was my friend's, but is also one of my greatest moments as a Tau player. I was playing Tau against my friend playing Dark Eldar. One of my Fire Warrior squads is getting cut up, starts to back off to put some distance between themselves and the enemy, only the Sgt and 3 warriors left, when they get charged by a unit of Dark Eldar Reavers. A few rounds and some amazing dice rolling later, one single Reaver is running back across the battlefield, my lone Fire Warrior Sgt remaining and standing tall.
My Dark Eldar friend is still disgusted at that units rolling from that day.

The most embarrassing fail was having my hive tyrant get his butt kicked by a group of veterans that weren't armed with plasma or melta or melta bombs.
It was a squad of vets with 3 flamers and Yarrick. I shoot at them with my hive tyrant that has an armored shell (2+ armor save) and 2 twin-linked devourers. I kill 4 guys including all the flamers. I charge, the vets overwatch. 2 hits, 2 wounds, 2 failed armor saves, that's 2 6's, 2 6's, and 2 1's. I lost half my wounds just from overwatch.
Then some poor roles means I was stuck in combat for a while and when I got out I had 1 wound left. Then Yarrick stands back up and shoots my hive Tyrant in the butt and kills him. I was a sad panda.

Had a CSM havoc unit with 4 plasma guns, the sarge and a bolter armed havoc pop out and open fire at a nearby enemy unit. All 4 plasma guns fired twice...rolled *8* 1s...no more plasma guns when all was said and done. :( Reminded me of my best Epic failure where 10 of 12 cannon of khorne blew themselves up on the first turn of a 6k per side game. THAT was priceless...

Had a game the other day (my guard vs his tau) where he got the Warlord trait that gives him a VP for every one of my characters he kills in a challenge. So once a weaker flank opened up, his warlord started serially charging my smaller units. I did better than I expected (managed to cut through his shield drones and get him down to 1 wound after a few turns of this) but the problem was that I kept succeeding my morale rolls after losing combat. He'd inflict eight or so wounds, getting the squad down to where he could finish them off in another round, and I'd need a 2 or 3 to stay locked in combat. For once, my guardsmen were steadfast in their duty. Thus, on my turn I couldn't shoot his warlord, he'd finish off the unit in my assault phase, and then he'd charge another unit on his turn. I ended up losing, and my troops' inability to fail the one time I needed them to fail played a part.

I've posted this one in the past, but as someone was requesting terminator fails, I'll go ahead and post it again, because it was quite an epic fail.
Keep in mind, this was back in 5th edition, so some rules no longer apply.
So, it was me with my Chaos Space Marines versus my friend and his Tyranid. I rolled up my land raider filled with Abaddon and four khorne marked dual lightning claw wielding terminators, and just misjudge the distance to his squad of gargoyles, so I can't charge (Oh the days of no pre-measuring). His turn comes up, and he charges the gargoyles right into the terminators. Now, I am thinking I have this in the bag. They are just gargoyles after all.
Abaddon swings first with his Initiative 6. I roll a 1 on his daemon weapon, Abaddon proceeds to whack himself in the face instead of attacking the enemy gargoyles... "Okay... that's fine, I should still be okay with this combat" or so I told myself.
His gargoyles swing next, Initiative 5 due to FC if I remember right. After all his hits and wounds, 7 total got thrown on my terminators. No problem right? I'll fail one, maybe two, no big deal.
I rolled five 1's. All my terminators die except Abaddon. -.-
The following combat, Abaddon proceeds to tear them all a new one, however the failed turn of combat gave my friend time to bring swarmlord over with his lash whip wielding Tyrant Guard and finish Abby off after he finished the gargoyles.
Another example of terminator fail, again with Chaos -
I was in a two month long escalation league. During this league, I brought a small squad of terminators with combi-meltas to suicide the enemy HQ or what have you. Out of the 20+ games I played with this unit, they never once (no joke) managed to do anything with those combi-meltas. I saw more 1's with that unit than should have been conceivably possible.

I almost forgot to include my favorite fail I ever had the pleasure of witnessing.
Buddy of mine was playing with Orks. Thousand point doubles game. (4 players, 2v2, thousand points each). Ork player brought Ghaz, a squad of boys, and a squad of decked out nobz in a truck for fun. Enemy player goes first - Dark eldar player, first shot in the game, fires at the truck - Hit - Pen - Explodes. Ork player proceeds to roll on the Ramshackle chart and has to roll the truck some distance in a random direction. Now, at this point, we know this isn't possible, however at the time we didn't. The random direction and distance sent the truck off the table, assumed with the squad of nobz as well. In an attempt and true orkish glory, the player then sent ghaz into the fray against the closest enemy, just barely making the charge against Vulkan and a squad of 10 thunder hammer wielding terminators... He failed to wound a single model, and the terminators / Vulkan kill, i believe, 17 of his 20 boyz. He is now out of fearless range for Orks, and rolls his moral check, fails, and is below 25% so can't rally.
Ork player killed himself in round 1 of the game.

This was my mate who had the bad luck. He had his nice unit of cheesy thunder wolves all with storm shields and a thunder wolf lord out the front with a 2+ save rampaging through my IG lines. when a single unit of guard, 6 man strong, rapid fired. i got about 6 wounds, which was enough to take the last 2 wounds off and kill his lord who failed his look out sir and 2+, and take 3 wounds off of the rest of the squad(killing 2). his turn he charged the heroic guard unit with the thunder wolves i snap fired and killed the last thunder wolf.  laughed so hard!

And speaking of nurgle, I've had a 10-man unit of Nurgle marked Terminators with typhus go down to a conscript squad in one turn... (In 5th ed, so typhus wasn't quite as powerful, but terminators were retardedly more powerful, as they had a 4+ FnP...)

This past Sunday, my FLGS did 1500 point games using the Black Crusade mission from the Battle Missions book. In case you're not familiar, here's the highlights:
Straight kill points, no objectives/warlord/linebreaker
A destroyed non-vehicle unit goes into reserves, can come back next turn.
Non-fearless models get Stubborn and Preferred Enemy USR.
I figured I'd play for some lulz, so I took Trazyn to deny my opponent the satisfaction of killing him. I also took no vehicles, and no Fearless units, so that I could take full advantage of the mission rules. My list ended up being Trazyn, 3 Crypteks, 3 Lords, 1x5 Immortals, 2x5 Lychguard, and 2x12 Warriors.
I ended up playing DE, who brought ALL of his models in transports. They all had that thing that shortened my firing range by 6", so the Harp of Dissonance was about the only thing I got to shoot. Running at him was ineffective, because he'd just back up, then wipe the closest unit before they could get in range. I knew by the end of turn 1 that I'd made a pretty clear tactical mistake by not having range, and by having so many 1-man units.
I conceded in the middle of turn 5, with a score of 20-7.

2 weeks prior, we did 1000 points using the Blitzkrieg mission. Only Fast Attack units can be deployed at the start of the game, and can also enter from reserves on turn 1. The table next to me was Eldar vs. GK with an Eldar detachment. Eldar player went first, moved forward, but was out of shooting range. GK player went next, brought his Stormraven in from reserves, gunned down the Eldar player's warp spiders and used POTMS to destroy his Vyper. Then he used his own Vyper to destroy the Eldar player's Shining Spear. Spiders and Spears both failed leadership and ran off the table. Nothing left to shoot at, so end of turn... Eldar player is tabled on turn 1.

I'm a new 40K player. I heard a guy at the LGS was going to be playing Tau, so I swung by to watch some tactics. It was a 2v2 3,500pt game. Tau+Tau v Space Marines+Salamanders. I show up beginning of round 2. The SM player is sighting in his big gun, decides he has LOS and rolls to hit. Gets all 4 hits. I believe it was 11 per hit on 3 crisis, and 11 on some fire warriors. The one players rolls a "look out sir" than armor save for every hit, I think it was 22+ rolls, then the kid rolls for his fire warriors. Wounds are allocated and casualties removed. They are just about to start the next shooting group when the Tau player realizes that it was still a night scenario and he couldn't shoot that far....
The SM players were so demoralized.

Back in 5th a 1000pt game between my GK's and my friends guard. A 5 man squad of termies gets gunned down to 2 after failing 2+ saves, but no matter I'll charge those guardsmen and work my way through combat, I think. 2 termies charge, don't kill a single guardsman at S5 I6 and then both die in the first round after rolling 2 more 1s!
Edit: I went on to lose horribly, other fails that game included 3 turns of my whole army gunning at a chimera and not killing it

In one of my first games of 6th I was playing my Space Wolves + IG allies vs CSM (old codex) + Daemon allies at 3500 points.
I had a drop pod come down behind a defiler and figured with two plasma guns I had a decent enough chance of taking it out with shooting. I end up doing only 2 hull points. Fail to do any further wound points with the rest of the shooting that can target it.
Chaos turn, the defiler turns around and shoots at my drop pod. I'm completely baffelled but feel cocky that my opponent made a tactical error. My opponent whiffs, and I figure all is safe, until the assault phase comes the defiler assaults my drop pod. Again, I'm still a bit baffelled at the move but figure losing a drop pod is no biggie. The combat goes, and the defiler gets an explodes result on the drop pod. Rolls for inches:  . Rolls to wound, wounds all ten of them. I fail seven armor saves, beginning with wounds being allocated to the special weapons and standard that I had hidden behind my line of guys to avoid having wounds allocated to them. I had never thought of an explosion result resulting in the wounds coming to them directly.
In the end I was left with 3 grey hunters, no upgrades. Total fail of a drop pod!
Ended up losing that game, but that drop pod failure was the big fail moment!

Here's a fun one. Not my fail, but the other player's.
Apocalypse game under 5th edtion. Two thousand points per player, two Tau players on the opposing battlefield. Both take nothing but Hammerheads, Broadsides, and the occasional Remora.
Four thousand points of Tau heavy support open fire on my Baneblade for two turns, achieving precisely nothing.
Turn three, six genestealers assault and wreck said baneblade.
My fails... six Seraphim with a Palatine, two inferno pistols and meltabombs drop in behind a CHaos Baneblade and fail to scratch its rear armour with their three BS4 TL melta shots. Next turn, they get wiped out by a havoc squad with autocannons. ^^;

Just finished a game against Eldar with my IG. He was getting rediculously good rolls the entire game. As his last shooting attack in turn 5, he decided to throw a singing spear at my Valkyrie. Rolled a 6 to hit, 6 to wound. I rolled a 1 for a jink save. He rolled a 6 on the damage table. I started packing up.

I was playing a doubles match with my fiance against blood angels and plague marines. Furiouso drops in next to my grey hunters but can't charge from the pod. I'm thinking awesome! 2 melta guns in melta range! Roll the dice, both hit. Roll for the first armour pen, and rolled snake eyes. Same for the second roll... My grey hunters proceeded to get slaughtered by blood talons... That sucked...

This happened about a month ago. 3k of Necrons (me) vs a combined force of CSM and SM. Warpsmith curses my monolith during his turn 3. During my turn 4, Warpsmith is blown up by Imotekh's lightning. Fire the monolith this turn, or wait to fire it after it's no longer cursed? Heck, I have all four hull points intact, why not? Particle whip fires, hits the target, blowing up some noise marines, but roll a one for Gets Hot. Snap fire a flux arc at the same squad of marines. Another one. Two hull points left. Only other target is a Dev squad. With only two hull points left, I'm feeling kind of vulnerable, so I better shoot at them, right? Two more ones. Hulled my own monolith down in a single firing phase.
To make matters worse, it was on a Big Guns Never Tire.

In an apocalypse battle a ork player fielded the deffkopta formation (dont know the exact name). He got beaten in combat and then overrun by a lonely chaos lord killing 10 koptas. Only to realize later that they were fearless. But by then it was already to late to put them back up and his role was done.
Not really a fail but in the same battle a friend of mine had only one termie left against 20 gargoyles and 10 stealers. He had to make 20 saves and made every single one, after losing 4 termies to only the 10 stealers the previous turn.

I had one massive squad of thousand sons back in 5th ed with a sorcerer lord in them. They spent the whole six turns of the game head-butting a unit of necron scarabs, both the sorcerers in the squad also managed to kill themselves by perils of the warp by using their force weapons. I managed to win the match, but me and my mate kept on playing that combat once it was over to see if they could eventually beat the scarabs, they eventually did after turn 10+.
Also this game had one of my epic wins, when my daemon prince got charged by a unit of necrons, survived and wiped them out, charged a unit of necrons next turn, wiped them out. Spent an entire turn of necron shooting being pounded by gauss weapons, surviving everything with his 5+ invunerable save, before finally charging another squad of necrons and wiping them out, thus making them phase out of the game.
