Look up Wensaslao Moguel, it's quite interesting. He was shot nine times during his execution including a point blank shot to the head and survived.

Plot armor is necessary for any war story because, in a real war, any chump with a big enough gun can kill a hardened badass. Batman wouldn't be an entertaining read if someone managed to get a bullet into his face mask the first time he faced guys with assault rifles. What I personally do not like is when the protagonists wipe out entire armies all by their lonesome without taking a scratch. One of the reasons I never got into DBZ but that's just me.

At least in DBZ everyone died (at least once).

Might.
One in a million, nay even more than that they might. You might survive being hit by a 16 wheeler arctic truck coming at you at 120mph, doesn't mean you will.
As for the 'depending on where it hits', that depends on what we're talking about here. I assume a bolter round would be enough to rupture the internal organs of your average guardsman from the shock, though they'd probably survive a hit from a lasgun or an autogun, or any other such low-grade weapon.

Considering it's a mass-reactive shell that explodes when inside the target... It's basically a tiny missile launcher.
