Okay... big list. Obviously, there will be spoilers.
Everyone in Digital Devil Saga(2). Seriously, almost every important character ends up being stabbed, shot, eaten(which is actually fairly normal, considering the rest of the game's story), crushed, shocked, or just plain burned to death up. Worst of all, you have to watch each and every one of your party members have a slow and agonizing death scene.

The scene in "Shadow of the Colossus" where Agro the horse doesn't make it across that jump over the chasm. I played that level through again about 4 times, trying to convince myself that there must be a way to get him across.

For me, the Halo 3 Cortana bits, because even though she isn't dead, I was so sad that the poor computer chick was insane. I mean, it seemed like she was dead.
The other scene for me is Fable 2, when your sister dies in the beginning. It was so shocking.

Yeah, It was a good twist, but it was kinda crazy. I mean, killing a child in an RPG? I've never seen it before.

i was playing halo one back in the day and someone threw a plasma grenade at me and it hit my gun so i meleed a few times and the plasma flew off and landed on him. it was indead a 1 out of a million moment.

Personally my favorite moment was in World at War playing online. I set up a great headshot in cliffside and fired. Just as I fired, another guy ran into my shot. I swore at the fact that I just lost a headshot when all of a sudden it said +20. I was amazed when it said headshot and another kill.
However, both I and my original target were in prone and crouching stance respectively. So when the new guy ran in the way my crosshairs were directly over the guys crotch. I still claim it to be a double headshot.

If we're talking about memorable online kills, then I've got a great one.
I was playing a mod for Half Life called The Specialists. I don't know if you've heard of it, but it's a first person shooter that takes inspiration from The Matrix, Face/Off, and pretty much any Gun-Fu action movie (You can dive, flip off walls, superjump, pick up powerups to slow down time and dodge bullets, etc.). Anyway, it was a one-on-one deathmatch in a map that was a recreation of the Lobby scene in the first Matrix. My opponent came down and out of the elevator, and I was standing there waiting for him with a grenade in hand. I threw it at him. The grenade itself hit him, bounced off of his body, and he died. He didn't die from the explosion, he died from the grenade itself hitting him.
Playing around later, I found out that any game made with the Half Life engine causes one damage to a player if they're hit by a grenade. Even so, that has to be the most memorable frag of all time for me.

Some of the fatalities on Mortal Kombat are great. Sub-Zero's leg freeze-and-shatter come to mind, but nothing beats Kung Lao's hat buzzsaw.

I was playing Halo 3, waaay back then when it was new. The map was Guardian. Dude threw a plasma grenade into a man cannon on one side of the map. it landed on a fusion coil, which blew up, somehow launching the other one into the room with the other fusion coils, thus blowing up a grenade on the floor and killing me.

If you want a really sick MK fatality, try out Cyrax's internal grinder. He smacks the opponent around and then pulls them into this grinder in his chest and sprays the gore everywhere.

I was playing halo 3 one time and I was in the really sandy level with the elephants. I was plaing one on one and I was driving the brute chopper around and the other guy kept trying to get me with a gravity hammer. After killing him several times, I decided to drive into the pyramid with the gravity lift and got stuck in the top hole. He came up with the hammer and prepared to kill me when all of a sudden, the chopper freed itself and crushed the other guy.
