So, if it's the sergeant's toe that is sticking out, do you guys allow your opponent to kill him, too, if he's the only one visible?
If yes, then that means Telions and Vindicares skill to pick out targets out of a squad are completely useless in your group? Nah, wouldn't want to play like that.
And if you want a fluff explanation why the rules work like that...The one guy in the open gets hit, goes to ground, screams for his mama, some brave soldier scrambles out trying to pull him in, gets mowed down, too, his good friend Joe loses is nerves and runs out, rapid-firing in a futile attempt to hurt the bastards that killed Mike, gets killed, too...battles can work like that.
And, uh, for the topic: I've avoided the big mistakes luckily for the moment, however, I tend to forget to shoot my dreadnought's storm bolter. Could've saved his day one time...

They are not useless either way you play. If you can only see a sergeant and you can only shoot at that target, when you can see the whole squad (or more than just the sergeant), I think the ability to pick out targets from a squad is less than useless.

I was at a neighboring county's FLGS, and playing this guy I had never met. I had him in a tight spot, and I just finished carving him up in assault. We both know his unit is finished, and the game is going my way. I say 'Your turn,' and he doesn't respond. He just stares at me with this blank expression on his face. So I wait for about thirty seconds...of staring. I wave my hand and say 'Dude!'. He just stares at me, slack jawed, not saying anything. So after a minute or so of silent...open mouthed...glassy eyed...staring...I yell, 'Dude, WTF? Quit being a jerk and just roll the friggin dice!'
Thats when I realize everyone is staring at me. They (the guys who know him) come rushing over and checking to see if he is okay, calling me an donkey-cave, laying him down, calling his wife on the phone.

I made sure to set up the cityfight table before people came over to play. I even placed down all of the newly painted (well, stenciled) roads down. I did not, however, remember to put some barricades up to block the road that goes from long edge A to long edge B.
We, of course, end up deploying on long table edges. My sides opponent, playing orks, then proceeds to move his 19" along the road with his battle wagons, and charges the tau on our side on turn 1.
Perhaps a bit of a special move on his part for not pointing out the massive flaw in the table layout but, eh, I don't forget to block things off a bit anymore at least.

I once forgot that Termies can move and assault if they fire heavy weapons and rapid fire weapons. I spent an entire campaign not shooting at my opponent when i was within 12 charges and just trying off. Until someone reminded me that they could do it.
Oh and i once made a massive arguement that tanks have a leadership. Only to drag the entire FLGS into a pitted arguement about it. 5 minutes later i go, "opps my bad" and everyone just stared at me...

I tried out the IG special character, Captain Al'rahem. He's a platoon commander that requires his platoon to outflank and I decided to combine those three squads into one. After his platoon arrived they were later assaulted by my Space Marine opponent; the platoon lost combat and fled only to be wiped out.
Then in my next turn I attempted to shoot with the Captain against the SM's that wiped out his platoon only to roll a 'gets hot' and I thought that it instantly killed him. I must've got the 'got hot' rule mixed up with an older variant or just got it mixed up in general; he has more than one wound and should've stayed on the tabletop.
Not only did I miss that rule I also failed to field a Commissar in that combined squad which undoubtedly would've helped them keep their ground. Still a fitting way for the Captain to die since his whole platoon got wiped out.

I have had my moments of stupidity, but this is about what i did to one of my friends: he deep-struck his winged hive tyrant in front of a bunker containing my CCS hoping to blow it up. i had a leman russ demolisher nearby, so on my turn i decided to tank shock the hive tyrant for the hell of it. my friend attempted a death or glory, but forgot that his attacks hit the front armour. he failed to stop it, and ended up splattered against the hull of my russ the image of a hive tyrant descending from the skies and immediately getting creamed by a leman russ was hilarious!

Important lesson learned:
When firing grotzookas, make sure it is ABSOLUTELY IMPOSSIBLE for the template to scatter over the kanz.
A while back I was playing this guy's CSM and fired my kanz' zookas at a rhino that was about 8 inches away (only thing within range) the very first shot scattered right back at the kanz and landed square over the head of one...penetrate, blown up...

Well, I seem to occasionally reverse the movement and shooting phases and generally don't realize until I remark how 'long' the distances between models seem.
Then there's the picking up my army list and realizing I've forgotten to field a unit or two. Strangely enough, I don't get much sympathy in that scenario when I've just run the table on somebody.
