One time I forgot to field half my army. Big oopsie.

Actually, that has happened to me as well. (Well, I thought we were playing a 1000pt match while he fielded 2000pts. Only figured something wrong when I was toast by turn 2.

Another time I charged my space marine assualt serge (with sword and meltabomb) into combat with a ironclad dreadnought. It was at this point I learnt that in combat (unless immobilised) you need 6's to latch grenades onto a walker. Lady Luck was feeling pretty kinky that day so guess what I rolled with my meltabomb? That's right...

My Dreadnaught rolled a 6 and owned a wraithlord's face.

Thing's with t8 still scare me though.

Yesterday I charged my friends Carnifex with a lone Space Marine Sargent...I got a wound on it XD

I infiltrated a stealth team 18 inches away from a unit of genestealers, then moved towards them to bring their fusion cannon to bear. Perhaps i forgot to mention they were in a bunker (which we played as essentially a ruin, not an AV14 vehicle). every shot missed and I FORGOT TO JSJ AWAY. i killed one the next turn in CC before my super-super stealth squad went squish.

How 'bout the final game of a GT where I had won the previous 4 rounds.

Mission objective was to "destroy" 4 objectives that the opponent places for you to destroy. He who destroys more wins the game.

I blew my opponent off of the table by the end of turn 6.

...but forgot to destroy the objectives.

BTW, this was 4e so no "tabling=win" judge ruled a tie game.

Allowing a Broodlord with his GSs assault one of my SM squads ....never ever let that happen again.

Ok me and my two main opponents ones in no particular order:

1) Start of turn 6 "Hmm it's odd my obliterators haven't turned up yet... because I haven't actually rolled for them yet..."
2) Getting mixed up and playing 2 rounds of a 1750 pt tournament with 1500 points
3) Going to a tournament at warhammer world (ie a 4 hour drive) and realising you forgot to put the tank you were working on last night back in your case.

Weeellllll......My most major one would be choosing to play CSM based on their 3rd Codex only to find that the 4th Ed one was out after I had like 1000pts......*sob*....you Chaos players all know what I mean.

But seriously this isn't my moment but in like my 3rd ever game, my friend completly forgot about objectives. I had a Sorc and a Ksons Aspiring Sorc on an objective my only models left Vs about 3 10 man tac squads. I ended up winning 1-0 on objectives after he missed with 2 Lascannons, 6 Krak Missiles and about 30 Bolter shells! All did nothing for 2 turns! He was very vexed.....
I'll never forget that moment of ecstasy when I realised he had forgotten objectives!
