I have two:

Typhus vs Dread. Forgot in a moment of unbelievable rookidom that he'd be attacking the front armour. Last. With str 6. Bye Typhus!

Lord of skulls vs two knights. I feel this one wasn't entirely my fault, i was just so keen to paint up and use a KLOS that i didn't check the stats for the knights. They struck first, hit easier and had d weapons which did not discriminate between heavy and super heavy. Looking at the KLOS pts cost i figured it should be able to dispatch the knights with ease, being a manifestation of the god of battle and all.

First game after a few years, tried to Drop Pod my 5 man Sternguard Unit behind a Wave Serpent to try and Melta/flamer it to death. Drop Pod scattered the other side of the building so my guys couldn't do anything so I hid them in the ruins. Enemy turn, Wave Serpent comes round the corner and drops out 5 Dark Reapers.

He killed 4 Sternguard in his shooting, I then charged the Seargant in on my turn for a laugh and he died to Overwatch. Not a single shot was fired from that unit :(

First game I played ever with a friend who was just starting out like me, we thought cover and inv saves were applied after armor saves and it was Marines vs Marines and we had mostly Bolters...

Nothing happened the whole game!

Playing mostly Necromunda and Battletech, Malifaux is awesome too!

I've lost the count of the number of models I've assembled (and sometimes painted) without reading the rules first, only to find that I had given them the wrongest/worst combination of wargear possible.

Besides this, back in 5th when I got back to 40k, my brain refused to accept that Deffkoptas had two wounds.

Spuj - Dark Reapers - or anyone who joins their unit - actually can't overwatch.

(Also, their ignore-jink doesn't ignore any other cover saves, but it sounds like he flanked the ruins)

Dropped my Hive Crone down into glide mode on its first time out against Orks so I could flame a truck with the Drool Cannon. I forgot that Orks can Waagh! and move, run, charge. It got charged and killed outright by 10 Boyz, killing 2 of them in exchange from overwatch, but nothing else. I could have just flown it off the board, come back next turn, and killed the whole mob...

Well, I've never had a bad experience with 40k, but this reminds me of the first time I used a bunch of new Tomb King models in one game in fantasy. I rolled a good 9 misfires out of the 10 times I rolled the artillery die. Needless to say most of my stuff died without doing anything.

Mine are mostly in the melee phase, I will always forget something. In my most recent game I totally forgot warhymns existed when my guard blob got tied up in combat, to make matters worse I kept forgetting to cast hammer hand on my TH inq, and that smash made Straken's attack ap 2.
Had I remembered those I think I would have won by a much larger stretch than I did. (Tactical objs and Mech vet obj grabber won the game for me ₩

First time using my brand new converted Lord of Change:
My team mate was playing Nurgle as primary (we regularly play 2vs2) and his warp storm struck my Lord of Change. Needed to make a Demonic Instability test on 3d6 and rolled 16. Next round, my team mate realised he could've rerolled the result with Fateweaver.

First time using my Heldrake:
Flew it towards a large group of Imperial Guardsmen, flaming six. The single meltagun in that squad got a lucky snapshot off and downed my drake.

Typhus vs. Dread!!!! I did this too! Ha ha against my buddies contemptor on turn 2....derp.

Most recently playing my buddies tyranids w/ Daemons I mass charged his Warlord and Bodyguards with 6 seekers, 3 nurglings, and a squad of plaguebearers lead by a Nurgle Herald....He challenges, and without even thinking I of course accept....SO STUPID! We were locked for a couple of turns while all my other units just stood there... (FACE-PALM) ha ha I now know to think before blindly accepting challenges!

It was his new model, but I still figured it was derp enough to share

It wasn't his fault. He actually bought a special one off kit that represents a Monolith that's come out the wrong end of a warp storm... s'all guud.

In one of my first real games (I collected Tyranids long before I got into Chaos, but only played them maybe twice), I took Chaos Marines with the Mark of Tzeentch because Tzeentch is my favourite. I assumed that invulnerable saves had to be taken after armour saves, otherwise a 6++ that you have to pay so many points for would be worthless! My opponent didn't know any better, so we went with it. It saved a single Marine from Ratling Snipers.

In my very first game with Chaos Space Marines, I thought Terminators were effectively Slow and Purposeful (I had misremembered how Slow and Purposeful was similar to Relentless). It was a 500pt game and my (first-timer) opponent had five Terminators, along with my Tzeentch Terminator Lord with whatever the Tzeentch Daemon Weapon was called. Deathscreamer? It didn't end up making a big difference, but still.

Forgetting Tervigons are Psykers.

In my very first game with Fortifications (my opponent had a Bastion), I completely forgot that they are like vehicles. My Chaos Terminators fired a full volley of combi-melta into it, then started to hit it with their chainfists, and I didn't know if the 2d6 penetration roll worked on buildings, so it was effectively invulnerable for the whole game.

This same game was my first experience with a flyer (my Heldrake), and I started it on the board. Many Space Marines died that day.

I always forget Soul Blaze.

I forgot that the fire raptor has 2 reaper autocannon batteries, that all its guns are twin linked, that the autocannon has turret fire and that it had missiles.

I thought it was utterly worthless. Then I played it again with the proper rules and I love it.

My painful mistake was not in the playing but in the assembling. The first time I put together a current-version Rhino I missed a piece of sprue flash when gluing the floor plate between the two body sides. As such the whole thing ended up about an 1/8 of an inch too wide. It didn't occur to me that this happened until I went to put to the top on and it didn't fit. I use superglue as opposed to plastic glue so I was able to eventually break it apart and reassemble it, but I shattered the bulkhead wall, and the floorplate in the process. Several Rhinos later and I ALWAYS double dry-fit those floor plates now.

Oh, god, a similar thing happened to me with my vendetta.

Somehow, my brain decided that it was going to turn off spatial awareness during the entire construction, and I ended up somehow concluding that I needed to saw off a section of the underside of the vendetta' cockpit in order to make the buggering thing fit the hull. A good chopping spree (and several refit attempts) later, I realized my mistake, and that now I had a massive fething hole on the underside of the valk. Luckily, there was blu-tac to hand, and I treated it like greenstuff and re-plated the gap. After painting it over thoroughly and making sure that I never let anyone pick the flier up, not one person has notices the mistake. Despite the obvious bulging of the tac that was left.

Done that to a rhino, I had one side a tiny bit out, now all plates did not fit n it looks well a mess.

Done some work to salvage it but it remains the rhino that never quite was perfect.

Not so much with new model but with new edition, on one of mine and a mates first games of 7th i was hammering my mates space marines (khorne lord with aobf ripped apart his chapter master and bike squad) i was well ahead in points he had no chance of pulling back the game points wise, but i was rolling bad and he managed to take out my whole army bar 1 rhino by the end of turn 7 he had no shooting in range that could take it out. That was it i broke my losing streek yay i thought but made the fatal mistake of not making sure i would be out of max charge range after his movement, wouldnt you know he got 12 inch on charge and with last dice of last turn got a 6 n dam tabled me
