This is a little story about four people named Everybody, Somebody, Anybody, and Nobody.
There was an important job to be done and Everybody was sure that Somebody would do it.
Anybody could have done it, but Nobody did it.
Somebody got angry about that because it was Everybody's job.
Everybody thought that Anybody could do it, but Nobody realized that Everybody wouldn't do it.
It ended up that Everybody blamed Somebody when Nobody did what Anybody could have done.

I re-read this a few times and don't quite know where the issue is, but then again, I am a paramedic so I know the trade. A psychiatrist is not gifted with hind sight so how is this incompetant if the doctor felt he was no threat?
Obviously this will open up a can of worms, I have a feeling in my water.

As has been stated, psychiatrics is not an exact science. The pills that work for one person, might not work on another.
And what if he hadn't taken his meds? Still the 'incompetence' of health care professionals? Thing is, we will never know what triggered this, and hopefully none of us will know anything like the motivations of either party.
