Either constantly roling 2+'s for my Cannoness against a pile of instant death attacks-fething hate Blood Angles. ¬¬ Or rolling about 90% sixes for my squad of Witch Hunters Zealots when they foolhardedly charged my opponents tooled up death squad of Khorne Chosen, wiping them off the board and buggering up his plans something awful (every other he game blows them apart with his defilers now, as does every other one of my regular opponents ¬¬).

When my Warp Spiders were painted in camo to match my terrain and my opponent didn't see them sitting in the open. Gave them the opening they needed to get behind his lines and cause much slaughter.

Destroyed a Space marine battle company with a vindicator template weapon, rolled all 6s.
Next time this happened was agianst me. But this is what happened he had 3 baneblades shot at me. They missed and their templates landed on their deep strikers. Lol

This last weekend... something like 40ish shots at my necrons, passed almost every armor save, what I didn't pass, I rolled my WBB saves... seriously, made my opponent RAGE. I actually laughed a bit as I was rolling my armor saves and saw very few 1's and 2's...
After those rolls I felt like I should have just hopped a plane to Vegas  Mind you, I probably used up all my luck!

My Guard lascannon fired on my friend's Razorback. Rolled nothing but sixes. Ended up with a smoking ruin of a tank and a dead tcombat squad that his Sgt and a special weapon was in. All of this on the first shooting phase of the game. Not too shabby. He proceeded to loose horribly. Poor friend...

My friend destroyed my two Broadsides in a single shooting phase with his Land Raider in the begiining of the game. I got pissed and fired my Hammerhead at that raider. Rolled three consecutive 6's and blew it up. The resulting explosion also killed the Missile Launcher inside the raider and wounded the Chaptermaster in it.

While probably not the luckiest ever, in a recent game i had a platoon command squad get shot out of their chimera, then got the piss shot out of them, they go to ground for that 6+ cover in the open, only the platoon commander makes his 6+ cover against bolters and passes his Ld test to stick around. Next turn all that's shot at him is a stormbolter that whiffs. Last turn of the game he gets up, moves 6", then runs 6", contests the objective to tie the game, and the game ends. Twas awesome.
EDIT: just remembered probably the luckiest thing I ever managed. Back in 4th ed, was playing IG against DE, 1000pts. I had the first turn and miraculously managed to get LoS to my friends raider with a Leman Russ Battle Tank. I make the shot, hit with the battlecannon, penetrate, he fails his obscured roll to reduce to a glancing hit, and then I roll a 6. Back in 4th ed, a vehicle penetrated by an ordnance shot that then rolled a 6 for the result not only exploded, but was "Vehicle Annihilated". All passengers dead. This happened to be the raider which his tooled up Incubi squad and Shadowfield Archon.
Half his army dead in the only shot I make the first turn.
The game was called after that.

Might have been last week, when with Shrike i missed all 5 attacks (and the re-roll) when hitting on 3s.....

Actually, a few months ago, playing orks with TH/ SS termies in 2 Land Raiders (at 1500 points), for 3/4 turns i just moved the Land Raiders 12" one eventually died when Ghaz got a glancing hit, then next turn smashed it, one was being hit by 3 PK, the other by 2PK and Ghaz....

I have a few, all for different game types.
Favorite was this past week playing Planetstrike. On the first turn we took out 90% of their anti tank that could affect our Leman Russes (it was 4 on 2, 10k points per side). Only lost 3 snipers and my hellhounds for the 5 rounds we played. Our deepstrikers had also cleaned up inside the city pretty well, so game was in the bag when we called it.
On another 2v2 game, around 3k points, it was my Guard and my friends Orks vs two guard players. First turn he gets a trukk up in their face, and on their shooting turn EVERYTHING fired at it before it got wrecked. It really made up its points there.
And one that won the game for us, Me and friend's orkz again. This time against SM and IG (non mech). He had a weirdboy in a squad of 30. Power was teleporting. No scatter, dropped them all behind the enemy. Does his shooting, and gets ready to be annihilated. SM pulls pretty much everything back, and takes out a good chunk of the Orkz. Next turn, friend gets teleport again, no scatter. Pulls them back to our side. The SM pulling back allowed my hellhound to run forward, clear out the objective in 2 turns, and we took the point, winning the game.

2 SoB squads gang up and unleash Holy Retribution on Abaddon and his terminators buddies. They wipe out the Terminators and leave Abaddon with 1 wound. Next turn Abaddon charges one of the squads, rolls a 1 and fails his save.
the CSM player was not happy at all.

Recently, I was in a multi-player game, in which my Sanguinary Guard and Commander were just about to attack a Wave Serpent. However, my team-mate decides to destroy it via shooting first, denying me the chance to assault, and out of the wreckage come a 10 man unit of Howling Banshees with Exarch, and Executioner. I know my Guard are going to die anyway, so I thought "Feth it, I get the charge, maybe I can take a few out". I charge the Banshees, who strike first and get their Counter Attack bonus. At the end of the assault phase, there is one dead Sanguinary Guard, a slightly scuffed Commander, 10 dead Banshees, and a group of laughing players!

One of my friends had a lucky moment where a single ork rocket launcher immobilized my land raider, giving us a tie game and he won the tournament because of it. Needed a 5+ to hit, a 6 to glance, and a 6 to immobilize. Needless to say, he rolled 3 6s in a row. I was less than happy, because it meant a tie game (2 objectives, I had one that he couldn't touch, and my terminators in landraider ALMOST got to his objective and he had already thrown 60+ boyz at the squad and only reduced them by 1. I was out of range by 4 inches)

Reality is a nice place to visit, but I'd hate to live there.

In one game my jump chaplain hit a dread with a melta bomb and sploded it up, then my lone scout sergeant went to ground on the objective and sustained three rounds of shooting to win the game (He must have made himself really tiny while hiding).

I don't play a lot of games. But two moments stick out.
My CSM against vanilla marines
1st turn, my defiler fires off his battlecannon and kills 8 marines! The other two were right on the table-edge, so they had to retreat
Yesterday- My CSM against IG
A leman russ fired it's battlecannon, and killed eight of my word bearers. (Yeah, I know, same story). Next turn, my terminators deep striked and shot their meltas off into the side of that leman russ. It exploded! yay. That won me the game, more or less.

My luckiest moment was a game with my tyranids (4th ed), where one of my Zoanthropes saved himself with a warp field (6+ inv save  ) 3 times (thats 6 x 6s!  ) from perils of the warp (due to famous 3k6 psychic tests thanks to Eldar fancy helmets) and popped a tank in the same game  .
