Just flicking through the rulebook and found myself wondering who can actually equip Heavy Chainswords? (Page 60 in mini)

Might be for a new, unreleased codex (my money is on either SM or its the new Eviscerater for missionaries in IG, GK and SoB) may just have to wait a while and find out. Side note, the Legion Upgrade Pack for HH models on FW has a Heavy Chainsword bit.

I believe that the only character I know of that actually has a Heavy Chainsword equiped is Flesh Tearers Company Commander Gaberial Seth???

The heavy chainsword is actually an eviscerator, which is a weapon that priests and SOB use, course there's that one guy in the BA book who uses something similar, but worse.

HH Praetors, Consuls, Veterans and Sergeants can have them I think.

You should see some of the other Wargear kicking about for the HH legions. Paragon blades are just plain brilliant, I've had characters hide away from my challenges when I give my Praetor one

Seth has a unique item that is described as being a "massive, two-handed chainsword" but has no inherent special rules.

Completely untrue. Heavy Chainswords are just that. Eviscerators are much bigger. Also, in game terms, heavy chainswords are just S User AP 5 Melee. Eviscerators are S Userx2 AP1 Melee, Unwieldy, Armourbane.

Ordo Reductor Thallaxi can take them for 5 points in place of their usual CCWs as well, although since everything in HH has a 2+, 3+ or 4+/6++ save.... they're kind of pointless.

So it's in the 40K rulebook but can only be taken in the 30K ruleset?

It is simply a piece of wargear certain races or units can use. regardless if 30 or 40k they can be used in either

Currently, yes.
Although I think that SOB will actually get a lot of access to it when they get redone.
