I cant find the rules for Titan Close Combat Weapons (TCCW). I heard somewhere they deal 0-12 wounds or some nonsense, and im trying to determine what edition that was in.

Where would be the most current source? Maybe 30k?

No they work much like a normal CCW.
Only ones that deal more are destroyer type weapons.

Destroyer weapons are in the 40k rulebook bud.

Destroyer weapons can deal 0-12 Wounds (0 on a 1 on the Destroyer table, 12 on a 6 and another 6 to resolve the 6+d6 wounds a 6 nets you)...

Balanced Game: Noun. A game in which all options and choices are worth using.

Reaver and Warlord CCW rules are in the Tahgmata book. Effectively they are Str D, AP1. Some of the choices have Machine Destroyer and/or +1 Attack.

Not quite true. A D weapon can do 0, 2, 3, 4, 7, 8, 9, 10, 11 or 12 wounds.

Anything else is impossible, which is obviously intuitive.

What happens when the enemy GMC is reduced to t5? How does the instant death d3 wounds apply?

Per the BRB, Str D is treated as Str 10 for ID purposes. Can I assume this line of questioning is because you didn't read the actual rules in the book?

This is probably going to be my best bet for 30k rules.

Next Question : - Does adding a TCCW to a reaver grant +1 Attack for each one it has, in addition to the ones it comes with on its profile? does it Grant a bonus if I have two of the same weapon?

The answer to that, I'm afraid, is in the book.
